| Objective: pulmonary fibrosis is the common outcome of many interstitial lung diseases with different etiology.At present,the pathogenesis of this disease is unknown,and there is no effective treatment and drugs.Professor XX,my tutor,is good at using the method of warming and tonifying the kidney yang to treat pulmonary fibrosis.At present,many studies have shown that the drugs of warming and tonifying the kidney yang have the effect of anti pulmonary fibrosis,which provides a new idea for the diagnosis and treatment of pulmonary fibrosis.This experiment intends to further verify the rationality of its clinical application through the analysis of clinical data,and to explore the mechanism of wenbushenyang method on angiogenesis in rats with pulmonary fibrosis through animal experiments,hoping to provide a basis for clinical treatment of pulmonary fibrosis.Methods: to collect 55 cases of pulmonary fibrosis diagnosed by XX,director of the pulmonary disease department of the Affiliated Hospital of Shandong University of traditional Chinese medicine from September 2019 to December 2019,and the syndrome type of deficiency of lung and kidney from the outpatient medical records,and to establish a database.Based on the database,using Excel,SPSS and other software,to analyze the clinical application of the method of warming and tonifying the kidney yang taught by XX.36 rats were randomly divided into 6 groups: blank group(group A),prednisone group(group B),Xianlingpi group(group I),Xianmao group(Group H),Erxian group(Group K)and model group(Group x).The rats in other groups were fed with BLM solution for 2days.After the model was made,the corresponding drugs were given for intervention,and the blank group was not given any treatment.On the 14 th day after modeling,three rats in each group were randomly killed.The lung tissues of mice in the same position were taken from each group,and the appearance and pathological sections were observed,and the blood samples were taken.The remaining mice were killed on the 28 th day after modeling.The appearance and section of lung tissue were observed in each group.The microvessel density(MVD)and the expression levels of pigment epithelium derived factor(PEDF)and basic fibroblast factor(bFGF)in serum were measured by ELISA.Result:1.Clinical analysis: a total of 47 drugs were used in this study,with a total of590 times of using traditional Chinese medicine.Xianlingpi was the most commonly used Chinese medicine in this statistics.The drug analysis of 47 kinds of Chinese medicine showed that the application frequency of tonic drugs was the highest,and tonic drugs and tonic drugs accounted for the top two of tonic drugs.According to the 47 kinds of traditional Chinese medicine used,the first three of them are sweet,bitter and pungent,the first three of them are warm,cold and plain,and the first three of them are lung,kidney and spleen.2.Analysis of animal experimental data: on the 28 th and the 14 th day of the experiment,there was a significant difference in PEDF between hormone group and citronella group before and after the intervention,and the level of PEDF decreased significantly(P < 0.05).On the 14 th day of the experiment,compared with the blank group and Xianling spleen group,the expression level of bFGF in Erxian group decreased significantly(P < 0.05).Compared with Festuca group and Xianling spleen group,bFGF level in hormone group and model group increased significantly(P < 0.05).Compared with14 days,the expression level of bFGF in Erxian group on 28 days was significantly higher than that on 14 days(P < 0.05),and the expression level of bFGF in Xianling spleen group on 28 days was significantly lower(P < 0.05).At the same time point,the expression of MVD in each group was compared as follows: compared with the blank group,the expression of MVD in pulmonary fibrosis model group was significantly and gradually decreased on the 14 th and 28 th day(P < 0.01).Compared with the model group,the MVD expression in hormone group and Erxian group decreased significantly(P < 0.01),and that in Xianling spleen decreased significantly(P < 0.05).Compared with Xianling spleen group,MVD expression in Erxian group decreased significantly(P < 0.01).On the 28 th day of the experiment,compared with the model group,the expression of MVD in the lung tissue of Xianling spleen group increased significantly(P < 0.05),and that of Erxian group increased significantly(P < 0.01).Compared with 14 days after the experiment,the MVD value expression of Xianling spleen group was significantly higher(P < 0.05),and that of Erxian group was significantly higher(P < 0.05)3.HE staining and Masson staining results:there was no obvious pathological change in the lung tissue of rats in the blank group at different time points.In the model group,the alveolar wall was thickened,the alveolar wall was stained with more blue collagen fiber hyperplasia,and there was obvious pulmonary interstitial fibrosis.Compared with the same period model group,the degree of alveolar inflammation and pulmonary interstitial fibrosis in Xianmao group,Xianlingpi group,Erxian group and hormone group decreased significantly.Compared with other drug groups,the degree of alveolar inflammation and pulmonary fibrosis in Erxian group decreased at different time points.Conclusion: the method of warming and tonifying the kidney yang is effective and commonly used in the treatment of pulmonary fibrosis.The method of warming and tonifying the kidney yang can reduce the degree of pulmonary fibrosis by regulating the related factors of angiogenesis,improving the microcirculation obstacle of pulmonary fibrosis,improving the pathological manifestation of pulmonary fibrosis,anticoagulation and other aspects.Xianlingpi combined with Xianmao as the representative drug of the method of warming and tonifying the kidney yang,compared with Xianlingpi single drug and Xianmao single drug The effect of Maodan and prednisone is more significant,and the effect of warming and nourishing kidney yang is more significant,which is more conducive to the treatment of pulmonary fibrosis. |
