ObjectiveUsing Roy adaptation model as a theoretical guide to construct a early adaptation intervention program for enterostomy patients with colorectal cancer And to investigate the effects of Intervention Based on Roy Adaptation Model on psychosocial adaptation,quality of life and incidence of complications of colostomy in colorectal cancer patients.MethodsGuided by the Roy adaptation model,the preliminary draft of the intervention was formed on the basis of literature analysis and patient interview,and the preliminary draft was revised through expert consultation and pre-test.Then a total of87 patients with colorectal cancer colostomy that met the inclusion and exclusion criteria in an A grade hospital in Zhengzhou was selected for this study.The control group received routine nursing,and the experimental group received intervention based on routine nursing.After the intervention,Ostomy Adjustment inventory-20,City of Hope Quality of life-ostomy Questionnaire(COH-QOL-OQ),Ostomy therapists or department nurses were used to evaluate the Ostomy complications.Results1.Comparison of psychosocial adjustment scores of patients before and after intervention(1)There was no statistically significant difference before the intervention between the experimental group and the control group(P > 0.05).After the intervention,the experimental group were higher than those in the control group(P <0.05)in total scores of psychosocial adaptation and its dimensions.(2)The experimental group after the intervention in psychological,social adaptation and total score all dimensions were higher than before,they have significant difference(P < 0.05).Patients negative emotions and adapt to the total score are higher than before in the control group,they have significant difference(P< 0.05),positive emotion,adapt to the social life has improved after intervention,there was no statistical significance difference(P≥ 0.05)or higher.2.Comparison of patients’ quality of life scores before and after the intervention(1)There was no significance different each dimension and quality of life scores(P > 0.05)between the experimental group and the control group before the intervention.After the intervention,the experimental group in physical health,mental health,social health and quality of life scores were higher than control group,the difference was significance(P < 0.05),the experimental group is higher than the control group in mental health,but there was no statistically significant difference(P< 0.05)(2)The quality of life and the dimensions of the experimental group were higher than those before the intervention,and the difference was statistically significant(P<0.05).There was no significant difference in mental health and mental health between the control group before and after intervention(P>0.05).The total scores of quality of life and social health were higher than before,and the difference was statistically significant(P<0.05).3.Incidence of ostomy complicationsThere was no statistically significant difference in the incidence of ostomy complications between the two groups before intervention(P > 0.05).After intervention,the incidence of ostomy complications in the experimental group was lower than that in the control group,and the difference was statistically significant(P< 0.05).ConclusionThe early adaptation intervention program based on Roy adaptation model can effectively improve the level of social psychological adaptation of patients with colorectal cancer undergoing colostomy,improve the quality of life of patients,and reduce the incidence of complications after colostomy.
