Objective:Perinatal depression is a kind of depression that occurs during pregnancy or after childbirth,It has been a hot topic in recent years,although there have been a large number of studies,most of them have focused on the general maternal population and focused on postpartum depression rather than prenatal depression,there is even less research on the special group of pregnant women with threatened preterm delivery,and even less attention is paid to the spouses of pregnant women.In this study,pregnant women with threatened premature labor and their spouses were selected as the research objects,the study took "Actor-Partner Interdependence Model(APIM)" as the theoretical framework to investigate the characteristics and influencing factors of prenatal depression in pregnant women and their spouses with threatened premature labor,and to explore the relationship between the depression status and coping strategies of pregnant women and their spouses,in order to provide a theoretical basis for clinical development of health education program centered on husband and wife.Methods:This study was a cross-sectional study,using the convenience sampling method to selecting pregnant women with threatened premature labor and their spouses from March 2020 to December 2020,the questionnaire survey was conducted on both sides of husband and wife within 24 hours after admission,using the self-designed General Situation Questionnaire,the Edinburgh Postpartum Depression Scale and the Simple Coping Style Questionnair to collect information,then assess the social demographic characteristics,depression status and its influencing factors.According to the EPDS score of pregnant women,they were divided into the depressed group(≥13 points)and the non-depressed group(<13 points)for statistical analysis.SPSS 25.0 software was used to analyze the data,and descriptive satistics,chi-square test,t test,spearman correlation analysis and multi-factor logistic regression analysis were used for statistical analysis of the survey contents.Results:1.A total of 210 questionnaires were sent out and 203 valid questionnaires were received,with an effective rate of 96.67%.Pregnant women were divided into groups according to EPDS score,including 152 in non-depressed group,accounting for 74.9%;51 patients in depression group,accounting for 25.1%;The incidence of prenatal depression in pregnant women with threatened preterm delivery was 25.1%in a general hospital of Jinan.2.Statistical analysis was conducted on the general data of pregnant women with threatened premature delivery,and the results showed that there were statistically significant differences in the gestational age,the distribution of average monthly income,number of pregnancies,previous history of fetal protection,and pregnancy complications between the two groups(P<0.05).3.The mean negative coping scores of pregnant women were significantly different between the two groups(P<0.05).Spouses’ mean score of positive coping,mean score of negative coping and total score of Edinburgh depression were significantly different between the two groups(P<0.05).4.There was a positive correlation between the Edinburgh depression scores of pregnant women and the Edinburgh depression scores of their spouses(rs>0,P<0.01).On the subject effect,the mean negative coping score of pregnant women was positively correlated with the Edinburgh depression score(rs>0,P<0.01),the mean score of spouse’s negative coping was positively correlated with spouse’s Edinburgh depression score(rs>0,P<0.01),the mean positive coping score of pregnant women was negatively correlated with Edinburgh depression score(rs<0,P<0.05);In the aspect of object effect,the mean negative coping score of pregnant women was positively correlated with the Edinburgh depression score of their spouse(rs>0,P<0.01),spousal mean positive coping scores were inversely associated with Edinburgh depression scores(rs<0,P<0.01),there was a positive correlation between mean spousal negative coping scores and Edinburgh depression scores(rs>0,P<0.01).5.Pregnant women were divided into non-depressed group(0)and depressed group(1).To whether depression as dependent variable,the average score of positive coping of pregnant women,the average score of negative coping of pregnant women,the average score of positive coping of spouses,the average score of negative coping of spouses,pregnant women,the negative response to average,spouses positive scored an average score,spouse,negative coping,Edinburgh depression scores of the spouses,BMI,gestational age,the number of pregnancy,previous history of fetal preservation,pregnancy complications,the average monthly income as the independent variables,multivariate logistic regression analysis found that the higher the mean score of spouses’ positive coping,the less likely pregnant women were to be depressed(B=-1.819,P<0.05);The higher the mean score of spouses’ negative coping,the more prone to depression(B=2.001,P<0.05);The older the gestational week,the less likely to be depressed(B=-1.046,P<0.05);Pregnant women with complications of pregnancy were more likely to be depressed(B=1.921,P<0.05).Conclusions:1.Gestational age,mean monthly income,number of pregnancies,history of pregnancy preservation and complications of pregnancy are the factors affecting the status of depression in pregnant women with threatened premature labor.2.The depression status of pregnant women with threatened preterm labor is related to their own coping strategies as well as their spouse’s depression status and coping strategies.3.Medical staff should pay attention to the depression status of women and their spouses with threatened premature labor,bringing both husband and wife into the nursing plan,and carry out family-oriented nursing intervention,so as to reduce the incidence of perinatal depression.4.It is suggested to pay attention to the prenatal depression of special pregnant women and develop dynamic and multi-dimensional psychological assessment tools for the perinatal period,so as to assess the psychological changes of each period more accurately.
