Objective To investigate sleep quality in pregnant women during their first and second and third trimester and to identify risk factors associated with sleep quality. Methods Data was extracted from Chinese Pregnant Women Cohort Study(CPWCS).A total of 1 751 pregnant women were included.Differences in sociodemographic information, living environment,health-related behavior, and depression were analyzed between normal&abnormal sleep duration group and between normal&abnormal sleep quality group, using chi-square test.Logistic regression analysis were used to explore the influencing factors of sleep quality of pregnant women. Results Among the 1 751 pregnant woman included in the study, rates of pregnant women sleeping≤7h and>9h were 24.20%&15.20%in the first trimester(5-13 weeks gestation,T1),25.50%&11.30%in the second trimester(14-27 weeks gestation,T2),33.4%&7.4%in the third trimester(>27 weeks gestation,T3).19.9%,19.30%and 27.00%experienced poor sleep quality in T1,T2 and T3,respectively.45.10%pregnant women suffered non-persistently poor sleep quality and 2.30%suffered persistently poor sleep quality during T1,T2 and T3.Pregnant women were generally suffered from subjective poor sleep quality, difficulties in falling asleep, sleep disorders and daily fatigue. And as gestation week went on,sleep quality of pregnant woman was getting worse and worse.≥30 years old, living in southern area of China, living in the city, highly educated and husband highly educated,in work and husband in work, home decoration within a year, poor room ventilation, irregular diet, passive smoking and depression were related to short sleep duration.Regular diet(OR=0.72,95%CI:0.53~0.99)in T1 was a protective factor of sleep quality in pregnant women during T1.Unplanned pregnancy(OR=1.34,95%CI:1.02~1.75),passive smoking(OR=1.47,95%CI:1.15~1.87)and depression(OR=2.82,95%CI:2.18~3.68)in T1 were risk factors.Underweight, overweight,in work, dissatisfied room ventilation, drinking alcohol were related to poor sleep quality in T2.Work(OR=0.75,95%CI:0.57~0.985)and regular diet(OR=0.49,95%CI:0.32~0.7)were protective factors of sleep quality in pregnant women during T2.Living in southern area of China(OR=1.35,95%CI:1.05~1.74),husband in work(OR=1.78,95%CI:1.05~3.19),sugar-sweetened beverages intake(OR=1.38,95%CI 1.05~1.79),passive smoking(OR=1.49,95%CI:1.16~1.91)and depression(OR=3.16,95%CI:2.44~4.12)were risk factors.Underweight, overweight, dissatisfied&poor room ventilation, irregular diet in T1,passive smoking in T2&T3 were related to poor sleep quality in T3,Regular diet(OR=0.64,95%CI:0.42~1.00;OR=0.55,95%CI:0.34~0.88)in T2 and T3 was a protective factor of sleep quality in pregnant women during T3.Living in southern area of China(OR=1.46,95%CI:1.17~1.82),sugar-sweetened beverages intake(OR=1.48,95%CI:1.16~1.87)in T2,depression(OR=1.31,95%CI:1.03~1.67;OR=2.74,95%CI:2.14~3.52)in T2 and T3 were risk factors.Persistent room ventilation(OR=0.82,95%CI:0.67~1.00)and persistently regular diet(OR=0.74,95%CI:0.59~0.93)were protective factors of non-persistently poor sleep quality in pregnant women. While living in southern area of China(OR=1.34,95%CI:1.10~1.63),passive smoking(OR=1.33,95%CI:1.06~1.67)and persistent depression(OR=1.88,95%CI:1.46~2.43)during pregnancy were risk factors.Persistent room ventilation(OR=0.43,95%CI:0.20~0.89)and persistently regular diet(OR=0.32,95%CI:0.17~0.60)were protective factors of persistently poor sleep quality in pregnant women. While living in southern area of China(OR=3.48,95%CI:1.21~10.00),passive smoking(OR=2.53,95%CI:1.26~5.09)and persistent depression(OR=3.08,95%CI:2.36~4.01)during pregnancy were risk factors. Conclusion The rate of poor sleep quality is high among Chinese pregnant woman during T1,T2 and T3.And the rate of non-persistently poor sleep quality is extremely high. The risk factors of sleep quality are multiple.Regular diet and work, reduction of sugar-sweetened beverage intake, reduction of tobacco exposure, alleviation of depression symptom may help improve sleep quality among pregnant women.
