| Objective:In the treatment of benign prostatic hyperplasia(Benign Prostatic Hyperplasia,BPH),the existing viewpoints and guidelines believe that individualized treatment should be carried out according to different prostate volume(Prostate Volumes,PV).Studies at home and abroad have proved that there is a significant correlation between the progress of benign prostatic hyperplasia and prostate volume,and PV has important guiding significance for the surgical treatment of BPH.Foreign studies have confirmed that the increase of prostate volume is related to many factors.at present,there are few studies on the relationship between free prostate specific antigen((Free Prostate Specific Antigen FPSA)and prostate volume in China.This study analyzed the relationship between PV and age,total prostate specific antigen((Total Prostate Specific Antigen TPSA)),free prostate specific antigen and f PSA/t PSA ratio in patients with benign prostatic hyperplasia to find a more simple and effective way to predict prostate volume.It has a certain guiding significance for initially guiding the treatment of patients with benign prostatic hyperplasia.Methods:Patients with abnormal PSA or lower urinary tract symptoms in the Department of Urology of Shandong Provincial Hospital from January 2017 to September 2019 were retrospectively collected.Patients with benign prostatic hyperplasia confirmed by pathology after ultrasound-guided transrectal prostate biopsy(Transrectal ultrasound-guided Prostate Biopsy TPB)or transurethral resection of the prostate(Transurethral Resection Of Prostate,TURP)were selected.A total of 427 patients with age,TPSA and FPSA levels,and ultrasound-calculated PV data were enrolled in the study.In addition to benign prostatic hyperplasia,patients with chronic prostatitis were included.According to the pathological diagnosis of chronic prostatitis,the patients were divided into two groups:simple prostatic hyperplasia group and benign prostatic hyperplasia group with chronic prostatitis.The correlation between PV and various factors was analyzed by Pearson and multiple regression,and then the(ROC)curve of subjects’ working characteristics was constructed.Calculate the area under the curve of different groups and find the best cut-off value,all the statistical results are p<0.05 as the results of significant difference.Results:In the population.PV was significantly positively correlated with age,TPSA,FPSA,and FPSA/TPSA ratio(r=0.211,r=0.347.r=0.561,r=0.372).Multivariate linear regression analysis was used to statistically analyze the influencing factors of each group.In the population cohort,free PSA was the only predictor of PV(P&L;0.001).There was no significant association between FPSA and PV in patients with chronic prostatitis,while the BPH alone group was the same as the overall cohort.i.e.,FPSA was the only predictor(P<0.001).ROC curves were constructed to evaluate whether free PSA predicted PV greater than 40 mL.age<60,60-70,>70,The results of lOng/ml and 10-30ng/ml PSA groups were 0.773,0.750,0.781,0.736,0.845,0.744,0.853,respectively.Conclusions:1.There was a significant positive correlation between PV and TPSA,FPSA in patients with prostate.Although there was a significant correlation between TPSA and PV,there was a stronger correlation between FPSA and PV,and there was a stronger correlation between、FPSA and PV in different age groups and different PSA segments.3.In patients with prostatitis,considering that there is a significant relationship between free PSA and PV only in univariate analysis,the predictive value of free PSA in such patients may be limited. |
