| Objective:Data mining method was used to summarize the common syndromes and syndromes combination distribution of cerebrovascular cognitive impairment,and to provide basis for TCM syndrome differentiation and early intervention.Methods:This study was a cross-sectional study,102 patients with cerebrovascular disease cognitive impairment were included in the outpatient and inpatient departments of geriatrics department of Shanxi Hospital of Traditional Chinese Medicine from January2019 to December 2020,and the filling of CRF form was improved.Based on the observation table,Epi Data3.1 software was used to establish a database,and SPSS26.0 was used to conduct statistics.The common TCM syndrome elements of this disease were summarized through factor analysis,and the combination and distribution law of the syndrome elements were summarized by clustering method.Literature review and the opinions of the director and deputy director of the department of the university were combined to obtain the distribution of TCM syndrome.Results:1.General data analysis: 102 patients with cerebrovascular disease cognitive impairment were found by statistical description,among which 51 were male,accounting for 50%;There were 51 females,accounting for 50%.The oldest and youngest patients were89 and 44 years old,with an average of 67.74±8.99 years old.A total of 84 patients were over 60 years old,accounting for 82.4% of the total number.In terms of occupation,the total frequency of workers and farmers is 39.2%.In terms of educational level,the frequency of illiteracy was 5.9%,the frequency of primary school was 22.5%,and the number of patients with junior high school or above accounted for 71.6% of the total number.In terms of lifestyle,smoking accounted for the highest proportion,up to 64.7%.In terms of past history,the top three with higher frequency were hypertension,diabetes and hyperlipidemia,of which hypertension accounted for 64.7%.In terms of family genetic diseases,hypertension,diabetes and cerebrovascular diseases were the most common,with frequencies of 73.7%,22.2% and 11.1%,respectively.In terms of the types of cerebrovascular diseases,subcortical white matter lesions,lacunar cerebral infarction and cerebral microinfarction had higher frequency,accounting for 59.8%,55.9% and 24.5% of the total patients,respectively.There were 47 patients in the mild cognitive impairment group and 55 patients in the dementia group.2.Analysis of the four diagnostic information: 102 patients with cerebrovascular disease cognitive impairment,the frequency of more than 25% of the symptoms are:Lumbar debility/pain,dizziness,blurred vision,night to urinate,acerbity,thirst,mouth pain,numbness,tine shakes,limb tired body weight,head weight,such as wrapping,spontaneous perspiration,headache,sticky mouth,weakness,shortness of breath,pee qing long,god is exhausted,cough while smiling enuresis,oral dark purple,irritability,chest distress,tinnitus,urine after Yu Li,face red,insomnia,sallow complexion,good heave a deep sigh,abdominal distension,limb pain,face Dark color,palpitations,five irritable heat,chills,hot flashes,night sweats,many dreams,clumsy limbs,short red urine,fat body,urgent urination,sputum;The tongue with frequency greater than 10% was dark tongue,red tongue,petechiae,light tongue,light red tongue,fat tongue and tortuous sublingual vein.The tongue coating whose frequency is more than 10% is greasy,white,yellow,thin and thick.Pulses with a frequency greater than 10% are chord pulse,slow pulse,fine pulse,smooth pulse and heavy pulse.3.Factor analysis:Factor analysis test was carried out on 102 cases of data.The KMO value was 0.586,and the significance of Bartlett’s sphericity test was 0.000,indicating that the data was suitable for factor analysis.Combined with the rubble diagram,the eigenvalue was selected to be greater than 1,the cumulative variance contribution rate was 75.089%,and the number of common factors was determined to be 20.According to the size of factor load,this study selected four diagnostic information factor load greater than 0.3,and extracted TCM symptoms contained in 20 common factors.They were named as F1-F20 respectively.Combined with TCM diagnostics,TCM internal medicine and clinical experience of the tutor,seven TCM syndromes of cerebrovascular disease cognitive impairment were obtained:Yang deficiency,fire deficiency,Yin deficiency,phlegm,blood stasis,qi deficiency and qi stagnation accounted for22.55%,18.63%,16.67%,16.67%,10.78%,8.82% and 5.88%,respectively.The disease site is related to the kidney,liver,heart and spleen.4.Cluster analysis:Clustering the 20 common factors,six types of TCM syndrome types were obtained:Yin deficiency and Yang hyperactivity,fire and phlegm;Kidney deficiency;Deficiency of liver and kidney,phlegm;Deficiency of spleen and kidney with blood stasis;Heart and kidney do not cross,mixed with blood stasis;Deficiency of the heart and spleen,phlegm and blood stasis.Conclusion:1.The symptoms of cerebral vascular disease cognitive impairment include Qi deficiency,Yin deficiency,Yang deficiency,blood stasis,phlegm,fire and Qi stagnation.The disease is located in the brain and is related to the liver,kidney,spleen and heart.2.The syndromes of cerebrovascular cognitive impairment were characterized by both deficiency and excess,and the pathological features were mixed with deficiency and excess.The syndromes of Chinese medicine are:(1)Yin deficiency and Yang hyperactivity,with fire and phlegm;(2)kidney deficiency syndrome;(3)deficiency of liver and kidney,phlegm;(4)deficiency of spleen and kidney with blood stasis;(5)the heart and kidney do not cross,mixed with blood stasis;(6)Deficiency of the heart and spleen,phlegm and blood stasis. |
