Objective:When patients with chronic obstructive pulmonary disease(COPD)are in the acute exacerbation stage, through the rational and sequential application of diversified traditional Chinese and Western medicine pulmonary rehabilitation technology, the clinical efficacy of this technology is evaluated,in order to reduce the rehospitalization rate, mortality and death rate of COPD patients. The economic burden of disease provides new methods and new ways. Methods:The 96 AECOPD patients who met the criteria for inclusion and discharge were randomly divided into 3 groups, namely, basic treatment group, basic pulmonary rehabilitation group, and combined traditional Chinese and western medicine pulmonary rehabilitation group. After enrollment, the three groups were given conventional anti-infection,phlegm and asthma supportive treatment, patients in the basic treatment group were only given supportive treatment, and the basic pulmonary rehabilitation group was given respiratory exercises and respiratory function training on the basis of conventional treatment(24 weeks of treatment)),skeletal muscle training(2 weeks of treatment),sports rehabilitation training(starting from the 14th day,22 weeks of treatment)and other basic pulmonary rehabilitation guidance treatments. The combined traditional Chinese and Western medicine pulmonary rehabilitation group combines traditional Chinese medicine preparations on the basis of basic pulmonary rehabilitation(treatment 2weeks),acupoint application(treatment course of 4 weeks),moxibustion(treatment course of 12 weeks),ear acupoint pressing beans and TCM fitness exercises(treatment course of24 weeks)and other diversified pulmonary rehabilitation methods sequential treatment, after 24 weeks of treatment,Observe the lung function of the three groups of patients, the number of acute exacerbations, the Saint George Questionnaire(SGRQ),self-efficacy energy scale(CSES),6-minute walking distance(6MW-D),dyspnea score(m MRC),COPD assessment test(CAT),Detect serum EO count,IL-6 and TNF-α.Results:The total effective rate of the pulmonary rehabilitation group with integrated traditional Chinese and Western medicine is 90.63%,the total effective rate of the basic pulmonary rehabilitation group is 83.88%,and the total effective rate of the basic treatment group is 73.42%.The overall efficacy of the pulmonary rehabilitation group of integrated traditional Chinese and Western medicine is better than that of the basic Pulmonary rehabilitation and basic treatment group(P<0.05);Basic pulmonary rehabilitation group and basic treatment group have no obvious advantages or disadvantages(P>0.05).At the12th week of treatment, there was no significant difference in the number of acute attacks between the three groups(P>0.05);at the 24th week of treatment, the number of acute exacerbations in the pulmonary rehabilitation group with integrated Chinese and Western medicine was significantly reduced, and the reduction was better than that of the basic pulmonary rehabilitation group, basic treatment group(P<0.05).The number of acute exacerbations in the pulmonary rehabilitation group of integrated traditional Chinese and Western medicine was reduced at the 24th week of treatment compared with the 12th week of treatment(P<0.05).The basic pulmonary rehabilitation group and the basic treatment group were compared with the 12th week of treatment at the 24th week of treatment. There was no significant change in the number of exacerbations(P>0.05).With the passage of time, the SGRQ score,CAT score,m MRC score,CSES score, serum immunological IL-6,and TNF-αof the three groups of patients gradually decreased, and the decline was most significant in the pulmonary rehabilitation group with integrated traditional Chinese and western medicine(P<0.05).The basic pulmonary rehabilitation group followed, and the basic treatment group had the smallest decrease. The 6-minute walking distance of the three groups gradually increased. The pulmonary rehabilitation group with integrated traditional Chinese and western medicine improved the most(P<0.05),followed by the basic pulmonary rehabilitation group, and the basic treatment group improved the least. With te passage of treatment time, the pulmonary ventilation function of the pulmonary rehabilitation group with integrated traditional Chinese and western medicine was statistically different at each time-effect point(p<0.05).In the basic pulmonary rehabilitation group and the basic treatment group, the pulmonary ventilation function FEV1/FVC(%)had no significant improvement in the 4th week of treatment compared with the 12th week of treatment(p>0.05),and the other time-effect points were statistically different(p<0.05);The percentages of FEV1and FEV1in the predicted value were statistically different within the group at each time-effect point(p<0.05).Conclusion:Pulmonary rehabilitation therapy combined with traditional Chinese and Western medicine can increase the 6-minute walking distance of patients with acute exacerbation of chronic obstructive pulmonary disease(phlegm turbidity obstruction lung syndrome),and reduce the St.George’s Questionnaire(SGRQ score),COPD assessment test(CAT)score,Dyspnea rating(m MRC)score,self-efficacy energy scale(CSES)score,IL-6 level,TNF-αlevel,and the degree of improvement was better than that of the basic treatment group and the basic pulmonary rehabilitation group. At the same time, combined traditional Chinese and Western medicine pulmonary rehabilitation therapy can improve patients’lung function and reduce the number of acute exacerbations. The combined pulmonary rehabilitation technology of traditional Chinese and western medicine can reduce the clinical symptoms such as coughing, phlegm and wheezing in patients with COPD,improve the quality of life and exercise endurance, reduce the number of acute exacerbations, and provide new ideas and new methods for the new model of pulmonary rehabilitation of traditional Chinese and Western medicine in the future.
