Objective:Cervical Spondylosis(CS)is a disease in which degenerative changes of the cervical intervertebral disc and its secondary pathological changes involve the surrounding tissue structure, resulting in a series of clinical manifestations. It is one of the common clinical diseases and frequently-occurring diseases.Neck type of cervical spondylosis(NCS)is an early pathological type of cervical spondylosis. The clinical symptoms such as neck pain, neck muscle stiffness, and limited neck range of motion caused by it are harmful to people’s normal work and life.TCM Tuina and Acupuncture,as traditional TCM therapies, are not only convenient and safe, but also have a good effect on painful diseases. In this study, guided by the basic theories of traditional Chinese medicine, biomechanics and human anatomy of western medicine, after analyzing the pathogenesis of cervical spondylosis, the treatment method of massage combined with acupuncture was selected, and a randomized controlled trial was used to compare fixed-point massage combined with surrounding acupuncture. The difference in clinical efficacy between the two treatment options of ordinary massage and ordinary acupuncture in the treatment of cervical spondylosis,to explore the optimal scheme of massage combined with acupuncture in the treatment of this disease, and to provide an effective reference for clinical choice of treatment options for neck type of cervical spondylosis opinion.Method:This study collected 64 patients with neck type of cervical spondylosis who were treated in the outpatient and inpatient departments of the Tuina Department of Wuhan Hospital of Traditional Chinese Medicine from October 2019 to October 2020.According to the principle of randomization, the patients were divided into an observation group of 32 cases and a control group. There were 32 cases in the observation group. The observation group was treated with fixed-point massage combined with surrounding acupuncture, and the control group was treated with ordinary massage combined with ordinary acupuncture, using the NPQ Neck Pain Scale,Mc Gill Pain Scale, cervical mobility and cervical spine dysfunction index(NDI)as an evaluation index, the comprehensive efficacy of the two programs in the treatment of cervical spondylosis was evaluated according to the evaluation criteria of cure, marked effect, improvement, and ineffectiveness.A follow-up was carried out one month after the end of treatment to evaluate the long-term efficacy of the two programs. The data collected in the research process were finally processed and analyzed by SPSS 20.0 statistical software. The count data were analyzed by chi-square test, and the measurement data were by t test. All statistical tests were performed by two-sided test.If P<0.05,the difference is considered statistically significant.Result:1.Equilibrium before treatment: there were no significant differences in gender, age, disease course,NPQ score,Mc Gill pain score, neck activity and cervical dysfunction index(NDI)between the two groups after statistical analysis(P>0.05),indicating that the two groups were comparable.2.Comparison of clinical efficacy: in the observation group, the number of treated cases was 32,the number of cured cases was16,the number of marked effective cases was 9,the number of improved cases was 5,the number of ineffective cases was 2,the total number of effective cases was 30,and the final effective rate was 93.75%;Control group treatment for 32 cases, cured cases of12,for 10 cases were markedly improved, better for 2 cases, invalid number of cases of 8,the total effective number of cases is 24,eventually effective rate of 75.00%,compared between two groups were eventually efficient through statistical analysis, statistically significant difference(P <0.05),the two groups were clinical curative effect, but the clinical curative effect of observation group is better than the control group.3.NPQ neck pain rating scale: the two groups after treatment1,treatment 2 period of treatment, treatment after 1 month of follow-up after the group comparison, statistically significant differences(P<0.05),the two methods of treatment can improve the NPQscore, comparisons between groups, differences are statistically significant(P<0.05),it shows that the immediate effect and long-term effect of the observation group in improving the NPQ score are better than those of the control group.4.Mc Gill pain scale: the two groups after treatment 1,treatment 2 period of treatment, treatment after 1 month of follow-up after the group comparison, statistically significant differences(P<0.05),shows that both treatments can relieve pain, comparisons between groups, differences are statistically significant(P<0.05),it shows that the immediate and long-term effects of the observation group are better than those of the control group in improving the Mc Gill pain score.5.Neck motion: In the intra-group comparison of the two groups after 1 treatment and 2 courses of treatment, the differences were statistically significant(P<0.05),and the difference was not statistically significant in the 1-month follow-up after the end of treatment(P>0.05).It shows that the two treatment methods can improve the neck mobility. In the comparison between the groups, the difference was statistically significant after one treatment and two courses of treatment(P<0.05).The difference was found in the follow-up 1 month after the treatment. No statistical significance(P>0.05),indicating that both the observation group and the control group can improve the range of neck movement after one treatment, but the immediate effect of the observation group is stronger than that of the control group. After two courses of treatment, the two groups of patients The range of neck movement tends to be normal, and the 1-month follow-up shows that the two groups have similar long-term effects on improving neck movement.6.Cervical dysfunction index: two groups after treatment 1,treatment 2 period of treatment, treatment group after 1 month of follow-up after the comparison, statistically significant differences(P<0.05),the two methods of treatment can improve the function of the neck, the comparison between the two way, statistically significant Differences(P<0.05),it shows that the immediate and long-term effects of the observation group are better than those of the control group in terms of improving neck function.7.Safety evaluation:During the course of this study, none of the patients had any major abnormal reactions.Conclusion:1.On the premise of comparability between the two groups, both fixed-point massage combined with encircling acupuncture method and ordinary massage combined with ordinary acupuncture method can effectively treat neck type of cervical spondylosis, but the clinical effect of fixed-point massage combined with surrounding needling method is more significant and the immediate and long-term effects are better.2.This study uses the basic theories of traditional Chinese medicine and Western medicine’s biomechanics and human anatomy as a guide, and adjusts the neck through fixed-point massage and acupuncture around the strain and pain points of the neck to improve the state of the neck muscles The small joint structure and local blood circulation improve neck pain, neck muscle tension and cervical joint mobility, thereby restoring the stability and biomechanical balance of the cervical spine to achieve the purpose of treating cervical spondylosis.3.The fixed-point massage combined with encircling acupuncture has significant clinical effects in the treatment of cervical spondylosis, and the operation is relatively simple, the patient’s acceptance is high, and the side effects are relatively small.Clinically,it can be given priority to use in the treatment of neck type of cervical spondylosis.
