| Objective By investigating and studying the clinical data,living habits and TCM syndrome data of 51 patients with severe acne in Wuhan area,the clinical characteristics of severe acne,bad living habits and TCM syndrome characteristics that may have an impact on the condition were analyzed to improve the severity of severe acne.The overall understanding of acne provides clinical evidence and ideas for clinical prevention and treatment of the disease.Methods In this study,a questionnaire survey was used to select 51 severe acne cases from October 1,2020 to January 31,2021 in the Dermatology Clinic of Wuhan Hospital of Integrated Traditional Chinese and Western Medicine and 51 cases of control from Hubei University of Traditional Chinese Medicine.In the group,a dedicated person was used to fill in the questionnaire one-to-one to assist the patients to complete the "Severe Acne Questionnaire" and "Severe Acne TCM Syndrome Collection Form" on the spot.Enter all the collected information into Excel to establish a database,and use SPSS20.0 software for relevant statistical analysis.Results1.Clinical characteristics:The age of patients with severe acne is 15-25 years old,the average age is 20.92±4.26 years,and the average course of disease is 4.20±2.68 years.62.75%of patients had family genetic history.The male to female ratio is 4.1:1,and the incidence of males is significantly higher than that of females.The most common clinical classification is abnormal acne(33.33%),followed by nodular acne(27.45%)and cystic acne(27.45%).The skin lesions are mainly distributed on the face(forehead,cheeks,jaw),neck,chest and back.There are seasonal differences in the patient’s condition,and most of them are worsened in summer and reduced in winter.92.16% of the patients believed that the worsening of the condition was related to eating spicy food,staying up late,and excessive stress.10 patients(24.39%)thought staying up late and eating spicy food,Mental stress can lead to aggravation of the condition at the same time;9 cases(21.95%)think that staying up late and eating spicy and stimulating food make their condition worse;13 cases(25.49%)think that eating spicy and stimulating food will aggravate the condition;7 cases(17.07%)think staying up late will aggravate the condition;2 patients(4.88%)thought that mental stress would aggravate the condition.60% of women in the severe acne group had menstrual abnormalities including menstrual cycle changes,decreased menstrual flow,or dysmenorrhea.60% of women in the severe acne group believed that menstrual cramps would aggravate the condition.2.2.Living habits: 78.43% of patients with severe acne have a daily habit of drinking milk or eating spicy and greasy sweets,smoking or staying up late,and they are more stressed and are more prone to negative emotions such as anxiety and depression.After Chi-square test,the living habits and mental factors of the severe acne group were significantly different from those of the control group.3.TCM syndrome analysis: cluster analysis results show that the location of severe acne mainly involves Shaoyang,Jueyin,Yangming and Taiyin.The pathological factors include cold,heat,dampness,blood stasis,and drinking water.The clinical syndromes are divided into Shaoyang Qi stagnation syndrome,Shaoyang Yangming syndrome,Yangming damp-heat syndrome,Taiyin syndrome,Taiyin water stop syndrome,blood stasis syndrome,and Jueyin syndrome.Conclusion Severe acne tends to occur in adolescent males.It is often accompanied by different lifestyle habits such as drinking milk every day,eating a preference for spicy and greasy sweets,staying up late,smoking,etc.,and is under high mental pressure,prone to negative emotions such as anxiety and depression.The location of severe acne mainly involves Shaoyang,Jueyin,Yangming,and Taiyin,and the pathological factors include cold,heat,dampness,blood stasis,and drinking water.The clinical syndromes are divided into Shaoyang Qi stagnation syndrome,Shaoyang Yangming syndrome,Yangming damp-heat syndrome,Taiyin syndrome,Taiyin water stop syndrome,blood stasis syndrome,and Jueyin syndrome. |
