| ObjectiveThrough the study and analysis of children with Kawasaki disease and complicated cardiovascular complications,the clinical significance of the ratio of mean platelet volume to lymphocyte in Kawasaki disease and complicated cardiovascular complications was explored,so as to provide a reference for reducing the long-term poor prognosis of children with Kawasaki disease.MethodCase-control study method,choose the children’s hospital in Shenyang in October2018-November 2020 admitted during the period of 60 cases of children with Kawasaki disease,Kawasaki disease group,2 days in the hospital early in the morning on an empty stomach condition collecting peripheral venous blood 2ml,detect and collect the results of routine blood including lymphocytes,mean platelet volume and calculated mean platelet volume and the ratio of lymphocytes.Venous blood was collected from children in the Kawasaki disease group,children in the Kawasaki disease group were given IVIG intravenous infusion at a dose of 2g/kg within 10 days during the course of disease.During the period of fever,children were given oral anticoagulant therapy with a dose of 30-50mg/kg.Children’s body temperature was stable for 3-5days,and the dose of aspirin was adjusted to 3-5mg/kg for oral anticoagulant therapy.Sixty healthy children who underwent physical examination in the Department of Child Health in the same period of time were included in the control group.On the day of physical examination,2ml of peripheral venous blood was collected,and the collected indexes were the same as those in the Kawasaki disease group.Group of children with Kawasaki disease in hospital 1day as a starting point,follow-up of 60 days,regular monitoring of heart colour to exceed 60 days,according to the result of a heart colour to exceed any abnormal cardiovascular system are divided into groups with no complications,complications analysis of mean platelet volume and mean platelet volume and lymphocyte ratio in the control group with Kawasaki disease group,complications and the significance of the changes in the group with no complications.SPSS25.0statistical software was used to analyze the data.The measurement data conforming to normal distribution was described as mean ± standard deviation(?x±s),while the measurement data conforming to skewed distribution was described as median(first and third quaternary)[M(P25,P75)],and the number of cases(percentage)was used to describe the counting data.Two independent samples t-test was used for measurement data conforming to normal distribution,Mann-Whitney U test was used for measurement data not conforming to normal distribution,and test was used for comparison of number data.Logistic regression analysis was performed on statistically significant data.0.05 indicates statistical significance.Results1.The results of Mann-Whitney U test showed that the mean platelet volume and mean platelet volume to lymphocyte of children in the Kawasaki disease group were increased compared with those in the control group,with statistically significant differences(P<0.05).2.T test results showed that mean platelet volume and mean platelet volume to lymphocyte were increased in the complication group compared with those in the non-complication group,with statistically significant differences(P<0.05).3.Unconditional Logistic regression analysis showed that elevated mean platelet volume to lymphocyte was a risk factor for cardiovascular complications in children with Kawasaki disease(P<0.05).4.The results of ROC curve showed that when mean platelet volume and mean platelet volume to lymphocyte were included in the diagnostic system of Kawasaki disease,the area,sensitivity and specificity under the curve were 0.772,0.417 and0.967 respectively0.778,0.580,0.697(P<0.05).mean platelet volume to lymphocyte was included in the diagnostic system of Kawasaki disease complicated with cardiovascular complications.The area under the curve,sensitivity and specificity were0.701,0.485,0.926(P<0.05).Conclusions1.The increase of peripheral blood mean platelet volume and the ratio of mean platelet volume to lymphocyte have guiding significance for the diagnosis of Kawasaki disease.2.The increased ratio of peripheral blood mean platelet volume to lymphocyte in children with Kawasaki disease has predictive value for complication of cardiovascular disease. |
