Furlow Reverse Double Z Flaps And Posterior Pharyngoplasty Clinical Effect Analysis Of Palatopharyngeal Incompetence After Cleft Palate Surgery

Objective: To study the clinical effect of furlow reverse double Z flap and posterior pharyngeal flap in the treatment of velopharyngeal incompetence after cleft palate surgery.Methods: 30 patients with velopharyngeal incompetence diagnosed after cleft palate surgery in our department were examined by nasopharyngeal fiberscope(NPF)before surgery.The speech intelligibility of patients could be evaluated by speech therapy Chinese articulation scale,and the duration of blister blowing was recorded.In strict accordance with the surgical indications,furlow reverse double-Z operation(lengthening soft palate to reconstruct levator veli PALATI)and posterior pharyngeal flap plasty(shortening pharyngeal cavity to achieve good velopharyngeal closure function)were used respectively.After 6 months of follow-up,the effect was evaluated,and the effect of different surgical methods was comprehensively evaluated through the evaluation of velopharyngeal closure function,speech intelligibility and blister blowing test.Results:after follow-up,there were 30 patients with good postoperative healing,surgical incision without infection.The patients were followed up again 6 months after operation to evaluate the recovery of velopharyngeal closure.The comparison results of each index showed that the preoperative and postoperative values of each group were all P < 0.05,and the difference of data study at this time was statistically significant,and the postoperative values were greater than the preoperative values.Conclusion: the speech intelligibility of 30 children has been improved in varying degrees.In clinical practice,patients’ velopharyngeal closure is evaluated by bubble blowing test,speech intelligibility detection and lateral cephalogram X-ray detection.The above three methods are convenient to operate and can combine subjective and objective,so they are widely used.Through the follow-up of patients with velopharyngeal closure,we found that furlow operation and posterior pharyngeal flap plasty played a significant role in the improvement of voice and velopharyngeal closure.
