ObjectivesTo construct an intervention program of yoga combined with meditation training for elderly patients with mild cognitive impairment(MCI)in the home community;To investigate the effects of the regimen on cognitive function,memory function,motor function and quality of life in elderly patients with home-based MCI,and to evaluate the feasibility and effectiveness of the regimen.MethodsThe literature content analysis method was used to preliminarily construct the intervention program of yoga combined with meditation training.The first draft of the intervention plan was revised after expert consultation.Eight MCI patients in the community who met the inclusion and exclusion criteria were selected for the pre-test,and further modified and improved to form the final draft of yoga combined with meditation training program.Ninety-six MCI patients who met the inclusion and exclusion criteria were selected as the research subjects in the area of the Hanghai East Road Community Health Service Center in Zhengzhou.The patients were divided into intervention group(n = 48)and control group(n = 48)by the method of coin toss.The control group received cognitive rehabilitation health education and community routine nursing,and the intervention group received cognition rehabilitation health education for 2 weeks and yoga combined with meditation training for 8 weeks on the basis of community routine nursing.Eight weeks before the intervention,intervention,intervention after 4 weeks the Montreal cognitive scale-Beijing edition(Mo CA-BJ),memory test Reviermead behavior second edition(RBMT-II)scale(POMA),Tinetti gait and balance test and determination of the world health organization quality of life questionnaire(WHOQOL BREF)evaluation of the scheme in elderly cognitive function in patients with MCI,occupy the memory function,the application effect of motor function and quality of life.SPSS26.0software was used to conduct descriptive statistics,chi-square test,Mann-Whiteney U rank sum test,t test,generalized estimation equation analysis and other statistical analyses on the research data.ResultsAmong the 96 subjects included in the study,2 cases in the intervention group and 1 case in the control group dropped out of the study halfway,respectively.A total of 93 subjects completed the study(46 cases in the intervention group and 47 cases in the control group).1.1.Comparison of scores of each indicator between the intervention group and the control group at three time points before intervention,immediately after 8weeks of intervention and 4 weeks after the end of intervention(1)The total score of MOCA,visuospatial and executive function,naming,attention,language,abstraction and delayed recall in MCI patients in the intervention group and control group were statistically significant in intergroup effect,time effect and interaction effect(P<0.05).(2)The total score of RBMT-II,story recall(immediate),face recognition,spatio-temporal date,appointment recall,story recall(delay),route recall(delay),letter recall(delay)in MCI patients in the intervention group and control group were statistically significant in intergroup effect,time effect and interaction effect(P<0.05);The recall route(immediate),recall letter(immediate)and spatiotemporal orientation score had statistically significant differences in time effect and interaction effect(P<0.05),there was no statistically significant difference in the effect between groups(P>0.05).(3)The total score of POMA,balance test and gait test scores of MCI patients in the intervention group and control group were statistically significant in the intergroup effect,time effect and interaction effect(P<0.05).(4)The total score of the WHOQOL-BREF scale and its various dimensions,including physiological field,psychological field,social relationship and environmental field scores,had statistical significance in the intergroup effect,time effect and interaction effect(P<0.05).2.Pairwise comparison of each index between the intervention group and the control group at three time points before intervention,immediately after 8 weeks of intervention and 4 weeks after the end of intervention(1)Immediately after 8 weeks of intervention and 4 weeks after the end of intervention compared with before intervention showed that the total score of MOCA,visual space and executive function,delayed recall and attention scores were significantly increased,with statistically significant differences(P<0.05).The naming score was higher after intervention than before intervention,with statistically significant differences(P<0.05).RBMT-II scale score,recall the story(immediate)(immediate),memory,memory route letters(immediate),date of space and time,recalled memories,stories booking(delay),the route to recall memories(delay),letter(delay)scores were significantly elevated,image recognition,image recognition,face recognition,temporal orientation score after the intervention is higher than before the intervention,difference was statistically significant(P<0.05);The total score of POMA and the score of gait test were significantly increased,and the score of balance test was significantly increased after intervention compared with before intervention,the difference was statistically significant(P<0.05);The total score of WHOQOL-BREF scale and the scores of each dimension were significantly increased,with statistical significance(P<0.001).(2)Four weeks after intervention and immediately after 8 weeks of intervention,the results showed that the total score of MOCA,visual space and executive function,naming and language scores decreased,and the difference was statistically significant(P<0.05).RBMT-II total score,story recall(immediate),route recall(immediate),face recognition,temporal and spatial orientation,appointment recall,story recall(delay),route recall(delay),letter recall(delay)were all increased,and the differences were statistically significant(P<0.05);The total score of POMA and the score of balance test decreased,and the differences were statistically significant(P<0.05);The total score of WHOQOL-BREF and the scores of each dimension were significantly decreased,and the difference was statistically significant(P<0.001).Conclusion1.Yoga combined with meditation training is a simple,easy to implement,safe and effective training program suitable for the elderly patients with mild cognitive impairment at home in the community,which has a positive effect on improving the cognitive level,memory function,motor function and quality of life of patients.2.As a kind of physical and mental exercise suitable for the physiological structure and sports characteristics of the elderly,yoga combined with meditation training program ensures the safety,scientific and feasibility of the training,which can be further promoted and applied in the community to provide reference for community medical staff to guide the cognitive exercise rehabilitation of elderly patients at home.
