Objective: To investigate the effect of early fresh breast feeding(FBMF)on the clinical outcome of preterm infants with gestational age ≤34 weeks;Methods: From January 2019 to December 2020,310 preterm infants whose gestational age was less than34 weeks and breast-fed were admitted to NICU in our hospital,divided into 83 cases of FBMF group and 227 cases of control group.FBMF was defined as the mother can provide fresh breast feeding 2 times a day or more,and continuously provide 14 days.The clinical data and delivery conditions of preterm infants were recorded,and the incidence of related complications,the average length of stay,the duration of parenteral nutrition,and the average weekly weight gain were compared between the two groups;Results: The weekly weight of FBMF group increased faster than control group [(87.96±50.15)g/(75.80±42.29)g,P=0.034],the incidence of feeding intolerance was lower [13(15.7%)/71(31.3%),P = 0.004].The duration of parenteral nutrition in the FBMF group was shorter than control group [(19.76±9.5)/(26.00±11.90)d,p = 0.005].The duration of mechanical ventilation was shorter than control group [(1.66 ± 0.67)/(6.7 ±2.28)d,p< 0.01],the incidence of feeding intolerance,NEC and BPD was low [7(18.4%)/23(38.4%)5(13.1%)/22(36.7%),6(15.8%)/24(40.0%)](P< 0.05).Conclusion: early fresh breast feeding for preterm infants with gestational age ≤ 34 weeks can increase body weight,reduce feeding intolerance,and is more beneficial to preterm infants with gestational age ≤32weeks,shorten the average length of hospitalization and reduce the duration of parenteral nutrition,reduce the risk of complications associated with preterm birth.
