ObjectiveTo explore the acceptance of Advanced Care Planning(ACP)in elderly patients with HIV/AIDS,and to analyze its influencing factors,in order to pr ovide scientific basis for the development of ACP in China.Methods1.Using the convenient sampling method,the 120 elderly patients with HIV/AIDS who met the criteria of inclusion and exclusion were selected f rom November 2019 to June 2020 in Jinzhou Infectious Disease Hospital,Panjin Infectious Disease Hospital and Shenyang Sixth People’s Hospital,due to the exclusion of death and other topics,seven people stopped an d refused to answer,113 valid questionnaires were finally collected.2.The self-designed patients’ basic information questionnaire,perceived social support scale,elderly death attitude scale and ACP acceptance qu estionnaire were used as the survey tools.3.Statistical method: Using SPSS22.0 software to process the data sta tistically.Measurement information is expressed in `x±s;The ACP acceptan ce scores of elderly HIV/AIDS patients with different basic conditions were compared by independent sample test or single factor method;The correl ation between death attitude,perceived social support and ACP acceptanc e of HIV/AIDS patients in hospital was analyzed by Pearson correlation;A CP multiple stepwise regression analysis was applied to the factors influen cing acceptance.Results1.The total death attitude score of elderly HIV/AIDS patients was(74.89±8.23),the total perceived social support score was(68.02±12.66),the t otal ACP acceptance score was(56.22±8.35).2.There were significant differences in ACP scores among elderly HIV/AIDS patients with different education levels and monthly income levels.3.The dimensions of death fear and anxiety(r=-0.598,P<0.01)and death escape(r=-0.837,P<0.01)in death attitude of elderly HIV/AIDS p atients,were negatively correlated with ACP acceptance,natural acceptanc e dimension(r=0.926,P<0.01)and approach-oriented dimension(r=0.402,P<0.01)in death acceptance were positively correlated with ACP acceptance,perceived social support and its dimensions(r=0.637~0.838,P<0.01)were positively correlated with ACP acceptance.4.The multiple stepwise regression analysis showed that natural accep tance,familiy support,friend support and death escape entered the regressi on equation,the regression equation was:ACP acceptance =36.565+1.177×natural acceptance +0.533× family support +1.555× friend support-0.493× death escape.Conclusions1.The ACP acceptance of elderly patients with HIV/AIDS is moderate,i ndicating the feasibility of developing ACP in this population.2.Natural acceptance and death escape in death attitude,family supp ort and friend support in perceived social support are important factors aff ecting the ACP acceptance of this group.Among them,natural acceptance,f amily support and friend support have positive correlation with ACP accept ance,death escape has negative correlation with ACP acceptance.3.While providing routine medical and nursing services for elderly pati ents with HIV/AIDS,medical workers should also disseminate ACP to pati ents and their families through various channels and in various forms,to make more elderly patients with HIV/AIDS establish a correct concept of d eath,understand and accept ACP,so as to carry out more in-depth explo ration and implementation of ACP.4.Families and friends should give more care,sympathy and support t o the group,communicate timely and effectively,understand their real end-of-life medical care needs,help and encourage patients to make good en d-of-life decisions.
