| Objective:To investigate the relationship between renal function and early clinical outcome in patients with acute ischemic stroke.Methods:A total of 426 patients with acute ischemic stroke hospitalized in the Department of Neurology of the first affiliated Hospital of Xinjiang Medical University from May 2019 to May 2020 were selected as subjects.The stroke severity was evaluated by NIHSS score on the day of admission.NIHSS score≤3 was defined as mild stroke group,and NIHSS score>3 was defined as moderate and severe stroke group.The recovery of early neurological function was evaluated by m RS score on the 7th day after onset,m RS score≤2 was defined as good recovery group,m RS score>2 was defined as poor recovery group,renal function was mainly evaluated by e GFR and albuminuria.Statistical methods were used to analyze the relationship between different levels of renal function and the severity of acute ischemic stroke and the recovery of early neurological function.Results:There were significant differences in the distribution of e GFR,proteinuria,creatinine,urea nitrogen,serum cystatin C and BMI between mild stroke and moderate and severe stroke,good recovery group and poor recovery group.Logistics regression analysis showed that e GFR<60ml/min/1.73m~2wasariskfactorforseverecondition(OR=17.308,95%CI:3.491~85.802,P<0.001)and early neurological dysfunction(OR=4.139,95%CI:1.069~16.022,P=0.040)in patients with acute ischemic stroke.Albuminuria is a risk factor(OR=2.707,95%CI:1.310~5.596,P=0.007)for the severity of acute ischemic stroke and has no correlation with the recovery of neurological function in the early stage of stroke.There was no clear correlation between the levels of creatinine,urea nitrogen and serum cystatin C and the early clinical outcome of patients with acute ischemic stroke.BMI may be a risk factor for severe illness(OR=1.148,95%CI:1.078~1.223,P<0.001)and early neurological dysfunction(OR=1.089,95%CI:1.025~1.157,P=0.006)in patients with acute ischemic stroke.Conclusion:e GFR<60ml/min/1.73m~2 may be a risk factor for severe illness and poor early neurological recovery in patients with acute ischemic stroke.Albuminuria may be a risk factor for stroke severity in patients with acute ischemic stroke,but has nothing to do with early neurological recovery. |
