Objective: To explore the changes in the volume of hippocampus and amygdala and the value of MRS in the lateral and location diagnosis of patients with temporal lobe epilepsy.Methods: In this study,50 patients diagnosed with temporal lobe epilepsy by EEG and clinical manifestations were collected as the observation group,and 30 healthy volunteers who were examined during the same period were selected as the control group,and all of them were subjected to bilateral hippocampal volume measurement and hippocampal MRS examination.Bilateral hippocampal volume measurement and hippocampal MRS examination were performed to compare the diagnostic value and detection rate of the two methods for temporal lobe epilepsy.Also performed amygdala volume measurements in patients with temporal lobe epilepsy who had negative hippocampal volume measurements to observe whether amygdala volume changes were associated with the onset of temporal lobe epilepsy.Results: The hippocampal volume of the affected side of TLE patients was significantly smaller than that of the contralateral side and the control group,and the difference was statistically significant(P < 0.05).The detection rate of hippocampal volume measurement for TLE diagnosis was 58.0%.The ratios of NAA /Cho/Cr,NAA/CHO and NAA/Cr in the affected hippocampus of TLE patients were significantly lower than those in the contralateral and control groups,with statistical significance(P < 0.05).The detection rate of MRS for temporal lobe epilepsy was 70.0%.The sensitivities of NAA/Cho,NAA/Cr,NAA/Cho+Cr for the diagnosis of TLE were71.7%,88.3%,and 95%,respectively,and the specificities were 66%,52%,and 52.9%,respectively.The sensitivity of hippocampal volume measurement for the diagnosis of TLE was 81.7%,and the specificity was 53.1%.The combination of hippocampal volume measurement and hippocampal MRS could improve the detection rate of temporal lobe epilepsy,and the combined detection rate of the two methods was 82%.Eleven cases of temporal lobe epilepsy with amygdala hypertrophy were found among temporal lobe epilepsy patients with negative hippocampal volume measurements,and 6 cases were consistent with the clinical diagnosis.And the detection rate for temporal lobe epilepsy patients with negative hippocampal volume measurements by amygdala volumetry was52.4%,and the coincidence rate was 54.4%.Conclusion:Both hippocampal volume measurement and hippocampal MRS examination can effectively diagnose temporal lobe epilepsy,and the detection rate,sensitivity,and specificity of hippocampal MRS examination are higher than those of hippocampal volume measurement.The combination of the two methods can be obvious Improve the diagnostic efficiency of diseases.Amygdala hypertrophy may be one of the causes of temporal lobe epilepsy.Therefore,in clinical diagnosis,attention should be paid to whether patients with temporal lobe epilepsy have amygdala hypertrophy.
