A Multiple-Factor Analysis Of Postoperative Visual Acuity Among Idiopathic Epiretinal Membrane Patients

Objective: To probe into the factors that influence the postoperative vision recovery by collecting the preoperative and postoperative clinical materials of patients with idiopathic macular epiretinal membrane(IERM for short).Method: 43 cases(namely 43eyes)of IERM patients who were diagnosed and underwent 23 G vitrectomy with ILMP(internal limiting membrane peeling)at the First Affiliated Hospital of Xinjiang Medical University between Jun.,2017 and Jan.,2020 were retrospectively collected;the postoperative follow-up lasted one year,in order to observe the patients’ preoperative and postoperative BCVA,EZ integrity of the surgical eye and the lateral eye,CT and NRT in macular region,and thus analyze the influence factors related with visual prognosis.Result: The differences of patients’ preoperative and postoperative BCVA,preoperative and postoperative surgical eye CT and NRT in macular region were all with statistical significance(P<0.05).The differences between patients’ preoperative surgical eye and lateral eye CT in macular region were statistically significant(P<0.05).The course of disease and postoperative BCVA changes presented negative correlation(r=-0.307,P= 0.045),EZ integrity and postoperative BCVA presented positive correlation(r = 0.395,P= 0.009),the preoperative CT in macular region had no correlation with patients’ BCVA changes(P= 0.81).The logistic regression between course of disease and postoperative BCVA established(B=-0.09,P= 0.045).Conclusion: 1.One year post vitrectomy with ILMP surgery,macular edema has apparent alleviation,the BCVA of IERM patients has effective improvement;2.There exists differences between surgical eye and lateral eye CT in macular region among IERM patients,which prompts that the reduced macular region blood flow is probably one important reason of the generation of IERM.3.The integrity of EZ affects the postoperative vision recovery among IERM patients;4.The shorter course of disease,the better postoperative BCVA recovery,which prompts that early surgical treatment is necessary for IERM patients in order to obtain a relatively satisfying curative effect.
