Objective:Investigation of the relevant elements of cognitive impairment at the acute cerebral infarction.It includes the bearing between TCM Syndrome Types and PSCI,which provides some clues for early prevention and intervention of PSCI.Methods:This study included 189 consecutive patients with acute cerebral infarction who were hospitalized in the Fourth Clinical Hospital of Xinjiang Medical University on January 1,2020 and January 25,2021 were enrolled consecutively.All the included patients were measured by the MOCA scale,among which 9 patients were excluded due to too much data missing,13 patients were unwilling to cooperate,and 167 patients were finally included.They were grouped according to the scores,54 cases were in the normal post-stroke cognition group(non-PSCI group)and 113 cases were in the cognitive impairment group(PSCI group).Collect case data between the two groups,including general information(BMI,age,education level,gender);History of atrial fibrillation,stroke,hypertension,coronary heart disease,diabetes;Serological examination results(Hb A1c,triglyceride,uric acid,plasma fibrin);Cranial imaging examination results(location of cerebral infarction,white matter degeneration,brain atrophy,dominant hemisphere infarction or not),TCM syndrome type.The data of the two groups were treated with KS test,t test,x~2test,u test,logistic single factor and multi factor regression.Results:1.In all of 167 patients with new onset cerebral infarction were fitted into this research.The course of disease was less than 14t,and 113 patients developed cognitive abnormalities,with a PSCI incidence rate of 67.7%;2.The difference between the two groups of cases,the degree of culture,uric acid,gender,WML,diabetes,hemoglobin,age,glycalated hemoglobin,brain atrophy,and homocysteine have statistically significant(P<0.05).Other Variables(BMI,plasma fiber protein,hypertension,low density lipoprotein,fasting blood sugar,triglyceride,stroke history,atrial fibrillation,superior hemisphere infarction)no statistical significance(P>0.05);3.Apply Logistic regression model for single-factor analysis,discovering academic qualifications,men,higher hemoglobin,more uric acid can reduce the risk of cognitive abnormalities in acute cerebral infarction(OR<1),high age,Diabetic history,high-glycized hemoglobin,high-profile cysteine level,WML,brain atrophy can increase the risk of cognitive impairment during cerebral infarction(OR>1).4.Logistic regression analysis showed that higher Hcy level(or=1.113,95%CI:1.003,1.235),older age(or=1.105,95%CI:1.02,1.196),higher glycosylated hemoglobin level(or=3.798,95%CI:1.51,9.552)and coronary heart disease(or=6.381,95%CI:1.1,37.02)were independent risk factors for acute ischemic PSCI(P<0.05).5.Statistical data analysis of TCM syndromes is not a relevant factor in PSCI.The traditional Chinese medicine syndrome type of cognitive impairment in patients with acute cerebral infarction was mainly wind-phlegm and stasis syndrome,accounting for 53.57%,followed by wind-fire disturbance syndrome,accounting for 25%.Conclusion:1.The incidence of cognitive impairment in the acute stage of cerebral infarction was 67.7%.2.Old age,high homocysteine level,history of coronary heart disease,and high glycated hemoglobin level are independent risk factors for PSCI in patients with acute cerebral infarction.3.In patients with acute cerebral infarction,the most common TCM syndrome type of cognitive impairment is wind phlegm stasis syndrome.
