ObjectiveThis study aims to evaluate the toxicity and efficacy of hypofractionated radiotherapy combined with chemotherapy in the treatment of SCLC patients,and to explore the feasibility of hypofractionated radiotherapy in the treatment of SCLC.MethodsAll patients in this study were informed of the risks of treatment and signed an informed consent form before radiotherapy.Medical records of SCLC patients treated in the Radiotherapy Department,First Hospital of Jilin University between July 2018 and October 2019 were collected.All patients treated with chemotherapy and concurrent hypofractionated radiotherapy(PTV: 30 Gy/10 F,PGTV: 45 Gy/10 F)were eligible for analysis.All patients received concurrent chemoradiotherapy.The chemotherapy was performed for 3-6 cycles by using etoposide plus eisplatin or carboplatin.Meanwhile,the indexes including ORR,if RR,if RFS and OS were calculated.ResultsThe medical data of 30 patients were eligible for analysis.The ratio of male to female was 19 : 11.The median age was 64.The median follow-up time was 17 months.During follow-up,the incidence of grade 3 radiation pneumonitis was 6.7%(2/30),and there were no grade 4 radiation pneumonitis or pneumonia-related deaths.The incidence of grade 3 radiation esophagitis was 6.7%(2/30),and there was no grade4 radiation esophagitis or esophageal fistula.The incidences of severe hematologic toxicities(grade 3/4)including leukopenia,granulocytopenia,thrombocytopenia and anemia were 33.3%,66.7%,26.7% and 16.7%,respectively.The overall objective response rate was 100%.5 patients exhibited complete response(16.67%),22 patients exhibited partial response(73.33%),respectively,and 3 patients(10%)achieved stable disease.The if RR was 13.3%(4/30)with a median if RFS of 28 months(95% CI:20.6-30.4)and a median OS of 29 months(95% CI: 26.7-31.3).Distant metastasis remained the main failure pattern.The brain was the most frequent remote failure site,followed by bone and liver.ConclusionThe toxicity of SCLC hypofractionated radiotherapy is tolerable,and these toxic reactions can be alleviated by symptomatic treatment.At the same time,the hypofractionated radiotherapy has a good local control rate,which can shorten the total treatment time and show a good effect.Hypofractionated radiotherapy can be considered as a treatment option for SCLC patients. |
