| Objective: Observe and compare the differences in general data,body mass index,history of smoking and drinking,medication,clinical manifestations,disease characteristics,and experimental indicators of UC with and without Epstein-Barr virus infection,and retrospectively analyze the differences in UC combined with Epstein-Barr virus infection Clinical characteristics,looking for relevant indicators that can predict UC combined with Epstein-Barr virus infection,to achieve early prevention of opportunistic infections,improve the prognosis of UC patients,and improve the quality of life.Methods: Selected from January 2017 to October 2020,139 inpatients with UC who underwent blood EBV-DNA testing were selected from the Department of Gastroenterology,First Hospital of Jilin University.All cases underwent colonoscopy and serum EBV-DNA testing,according to EBV-DNA Whether the test result is positive is divided into EBV-DNA positive group and EBV-DNA negative group.The clinical data are retrospectively analyzed,the clinical characteristics of the two groups are compared,and the risk factors of combined EB virus infection are analyzed.The statistical analysis of the data data adopts SPSS 22.0 statistical software.If the measurement data conforms to the normal distribution,the mean ± standard deviation(x±s)is used.The comparison between the two groups uses the t test.If the measurement data does not conform to the normal distribution,the medium is used.The digits and interquartile range are used to describe the trend of concentration and dispersion.The comparison between the two groups uses the rank sum test.The count data is expressed as the constituent ratio [n(%)],and the chi-square test or Fisher’s exact test is used.P<0.05 is The difference was statistically significant.Results: 1.There was no significant difference in gender distribution and BMI between the positive group and the negative group(P>0.05);the average age of the positive group was 49.05±14.58 years old,and the median age of the negative group was 41(29,54)years old.There was a difference in age between the two groups.It is statistically significant(P<0.05).Smoking and non-smokers in the positive group accounted for 17.7% and 82.3%,respectively,and smoking and non-smokers in the negative group accounted for 20.8% and 79.2%,respectively.The positive group drinking and nondrinking accounted for 17.7% and 82.3%,respectively,and the negative group drinking and non-drinking accounted for 10.4% and 89.6%,respectively.There was no significant difference in smoking and drinking between the two groups(P>0.05).2.There were 36 cases in the hormone treatment group,26 were positive,accounting for 72.2%,10 cases were negative,accounting for 27.8%,103 cases in the control group,36 cases were positive,accounting for 35.0%,67 cases were negative,accounting for 65.0%,the difference was statistically significant(P<0.05).There were 21 cases in the immunosuppressive treatment group,16 cases were positive,accounting for 76.2%,5 cases were negative,accounting for 23.8%,118 cases in the control group,46 cases were positive,accounting for 39.0%,72 cases were negative,accounting for 61%,the difference was statistically significant Significance(P<0.05).3.The common clinical manifestations in the positive group were diarrhea(57/62,91.9%),followed by blood in the stool(56/62,90.3%),abdominal pain(47/62,75.8%)and fever(23/62,37.1%),the common clinical manifestations of the negative group were diarrhea(68/77,88.3%),followed by blood in the stool(57/77,74.0%),abdominal pain(56/77,72.7%)and fever(15/77,74.0%).77,19.5%),the common clinical manifestations of the two groups were the same,and the difference in blood in the stool was statistically significant(P<0.05).The constituent ratios of the three clinical manifestations of vomiting,fatigue,and abdominal lymphadenopathy in the positive group were: 21.0%,22.6%,and 25.8%,respectively,and in the negative group: 5.2%,6.5%,9.1%,and the differences were statistically significant(P< 0.05).4.Comparing the positive group with the negative group,white blood cell(WBC),lymphocyte absolute value(LY#),aspartate aminotransferase(AST),alanine aminotransferase(ALT),γ-glutamyl transfer Increased levels of peptidase(GGT),erythrocyte sedimentation rate(ESR),high-sensitivity C-reactive protein(CRP),hemoglobin(HB),platelets(PLT),prothrombin activity(PTA),fibrinogen(FBG)The levels of blood potassium(K+),cholinesterase(CHE),and albumin(ALB)decreased,but only cholinesterase and albumin were significantly different(P<0.05).5.The median disease duration of the positive group was 37.5(6.5,86.25)months,and the median disease duration of the negative group was 39(8.5,73)months.There was no significant difference in the course of disease between the two groups(P>0.05).The severity of the disease was mainly severe,followed by moderate.The positive group accounted for 59.7% and 37.1%,respectively,and the negative group accounted for 49.3% and 39.0%.The lesions of the two groups were mainly E3,that is,extensive colon,positive Group and negative group accounted for 50.0% and 59.7% respectively.The endoscopy score of the positive group was mainly 3 points(67.8%),and the endoscopy score of the negative group was mainly 2 points(47%).The site and endoscopic scores were tested by chi-square test,and the differences in endoscopic scores were statistically different(P<0.05).6.The results of multivariate logistic regression analysis showed that age(P = 0.006),use of hormones(P = 0.001)and immunosuppressive agents(P = 0.010),endoscopy score 3 points(P = 0.040)are UC combined with Epstein-Barr virus infection Independent risk factor,albumin level is a protective factor for EB virus infection(P = 0.036).in conclusion:Conclusion: 1.Older UC patients who are treated with hormones and immunosuppressants are prone to EB virus infection.Gender,BMI,history of smoking and drinking,and the length of the disease have nothing to do with EB virus infection.2.Patients with UC combined with Epstein-Barr virus infection have blood in the stool,vomiting,fatigue,and abdominal lymphadenopathy,and serum examination is prone to lower albumin and cholinesterase.3.The endoscopy score of UC combined with Epstein-Barr virus infection is divided into 3 main points.Under the endoscopy,it shows severe inflammation,visible ulcers,and spontaneous hemorrhage on the mucosa. |
