| Objective:This study aimed to examine the root canal system and physiological foramen morphology of the maxillary first molar in the Chinese northeast population through Micro-computed tomographic imaging.It also aimed to evaluate the difference of the Xp-endo Finisher efficiency to reduce the root canal debris,smear layer,and bacterial biofilm over two different tapers prepared root canal.Methods:1.The root canal configuration,frequency of accessory foramina and lateral accessory canals,connecting canals,and physiological foramina shape and size of 30 maxillary first molar were investigated utilizing Micro-CT imaging and 3D reconstruction software.2.The teeth were then sterilized and invaded with Enterococcus faecalis to perform an infected maxillary first molar tooth root canal model.3.After confirmation of the infected root canal model’s proper modularization,the teeth were separated into two main experimental groups and one control group.The first experimental group teeth’ root canal preparation was done using Hyflex CM 0.4 taper files system followed by irrigation agitation using Xp-endo Finisher.The second experimental group teeth preparation was done using Hyflex CM 0.6taper files system followed by irrigation agitation using Xp-endo Finisher.The root canal irrigation was done using 3% Na OCl and 17%EDTA.4.Before and after root canal preparation,the bacterial colonization count was counted,and the teeth were examined under the SEM to observe the debris,smear layer,and bacterial biofilm removal.Results:1.The Micro-CT analysis reveals that the most frequent root canal configurations were 2-2-1/1(26.7%),1-1-1/1(20%),and 2-2-2/2(16.7%)in MB,1-1-1/1 in DB(100%)and P(93.3%).lateral accessory canals were observed 43.3% MB,36.6% DB,and 46.6% P.The isthmus connection between the MB canals was observed in 74.2%.One accessory foramen was observed 36.6% in the MB,20% in the DB,and25% in the P.The mean distance between the physiological foramina and anatomical apex was 0.65 mm in MB,0.69 mm in DB,and 0.75 mm in P.The physiological foramen shape frequencies were: 41.7%,25.2% and33.1%(MB);57.1%,28.6% and 14.3%(DB)and 47.4%,31.5% and21.1%(P)oval,round and irregular,respectively.The mean narrow and wide physiological foramina diameters(respectively)were0.21/0.34mm(MB),0.17/0.23mm(DB),and 0.30 m /0.38 mm(P).2.The bacterial clearance percentage using the Xp-endo Finisher in both taper groups(0.4 taper,0.6 taper)was by a range of 83.62%-87.40%.The scores of debris,smear layer,and bacterial biofilm in a 0.6 taper group were slightly higher than those in the 0.4 taper group,but there was no significant difference.Conclusions:This study provides detailed information about the root canal morphology of the maxillary first molar teeth in the Chinese northeast population.Moreover,it proved the absence of a significant difference in large tapered instrument efficiency over the less tapered one.According to results obtained in this investigation,the authors recommend a final physiological foramen preparation size using 0.4 taper files combined with Xp-endo Finisher irrigation agitation.However,study and clinical limitations must be considered when making a final decision in clinical endodontic treatment. |
