| Objectives:The prevalence of dens invaginatus, the corresponding treatment plan and prognosis of different types of dens invaginatus were analyzed by studying the CBCT imaging data of patients in the Stomatological Hospital of Jilin University from January2019 to December 2019,so as to provide the reference frame for clinical treatment.Three cases of dens invaginatus of different types were reported. Methods:A total of 3038 cases with CBCT were selected from Stomatological Hospital of Jilin University from January 2019 to December 2019.The tooth position, the type of dens invaginatus and the gender of the patients were recorded when the tooth enamel density images were observed from the crown direction to the square depression of the root or the formation of the root canal like structure accompanied by the main root canal.SPSS software was used to analyze the data of the prevalence and type of dens invaginatus and gender distribution of the patients. The clinical data, imaging findings and pathological data of 3 patients with dens invaginatus were collected to analyze the clinical and imaging characteristics of patients with dens invaginatus in different positions, and the clinical diagnostic criteria and treatment of such cases were summarized.Rank-sum test and chi-square test showed that when P<0.05,there was significant difference in the data, which was of statistical significance. Results:In the CBCT data of 3038 eligible patients,90 patients with dens invaginatus were found, and the prevalence of dens invaginatus in the population was about 3.00%.There were 48 cases of bilateral dens invaginatus,42 cases of unilateral dens invaginatus,39 male patients, accounting for 43.33% of the total,51 female patients, accounting for56.67%.The total number of teeth was 67,543,of which 139 were affected by dens invaginatus, and the prevalence of single tooth was about 0.21%.According to Oehler’s classification, the number of Ⅰ type dens invaginatus teeth is 100,and there were 27 teeth with Ⅱ type dens invaginatus,2 teeth with Ⅲ(a)type dens invaginatus, while Ⅲ(b)type dens invaginatus was not detected.According to the group of tooth position, there were 11 cases of maxillary central incisor,4 cases of maxillary central supernumerary teeth,121 cases of maxillary lateral incisor,1 case of maxillary canine teeth,1 case of maxillary premolar teeth and 1case of mandibulary canine teeth.During the study period,a total of 3 cases accord with full standard was collected, respectively Oehler ’s Ⅰ,Oehler ’s Ⅱ and Oehler’s Ⅲ by Oehler’s classification. With using the methods such as root canal treatment, extracting and periapical inflammation of the teeth with dens invaginatus after root canal treatment significantly reduced, meanwhile, periodontal status has significant relieved after the removal of teeth with dens invaginatus. Conclusions:Dens invaginatus was not as rare as we learned before,so clinicians should be aware of its existence.Early clinical examination and imaging examination are the most important methods to improve the prognosis. If clinical symptoms have occurred, new techniques can be used to appropriately treat the affected teeth, which can improve the aesthetic and function of the affected teeth to a certain extent. The statistical results showed that there was no significant difference between the rank of teeth with dens invaginatus and the gender of patients, which is the same as the prevalence of teeth and the gender of patients.Dens invaginatus usually occurs in the maxilla, preferably in the maxillary lateral incisors.Type Ⅰ dens invaginatus of Oehler’s classification occurs most.. |
