Objectives: This study was conducted to evaluate the effect of the combination of Modified Chaihu Guizhi Decoction and Megestrol on the improvement of loss of appetite and IL-6 level in patients with cancer cachexia based on the theory of "Sheng Sheng" Qi,by comparing the efficacy indexes before and after treatment within and between groups.Methods: A total of 66 subjects were enrolled according to the case criteria,and were divided into a simple randomized grouping method into Modified Chaihu Guizhi Decoction combined with megestrol group(treatment group,33 cases)and a megestrol group(control group,33 cases)for 2 courses(14 days in total)of clinical treatment.The study was carried out by comparing the baseline data and appetite VAS score,KPS score,TCM evidence score and IL-6 level before and after treatment,as well as correlation analysis of IL-6 level and appetite VAS score and cachexia stage before treatment.Results: 1.Comparison of baseline data: There were no statistical differences in age,height,weight,BMI,tumor type,gender,and cachexia stage between the two groups(P>0.05),which were comparable;there were no statistical differences in appetite VAS score,FAACT A/CS-12 score,KPS score,and TCM evidence score before treatment between the two groups(P>0.05),which were comparable;before treatment between the two groups There was no statistical difference in RBC,HGB,BUN,WBC,ALT,AST and Cr safety indexes between the two groups before treatment(P>0.05),which was comparable.2.Comparison of efficacy indexes: appetite VAS score: both groups improved after treatment than before treatment(P<0.05),but the treatment group improved more significantly(P<0.05),and the total effective rate of the treatment group(90.00%)was also KPS score: both groups improved after treatment(P < 0.05),but the treatment group improved more significantly(P < 0.05),and the total improvement rate of the treatment group(83.33%)was better than that of the control group(46.67%)(P < 0.05);Chinese medicine evidence points: both groups reduced their points after treatment(P < 0.05).IL-6 levels and correlation analysis: IL-6 levels decreased in both groups after treatment,and the decrease was more significant in the treatment group(P < 0.05);IL-6 levels and cachexia there was a positive moderate correlation between IL-6 level and cachexia stage;there was a negative weak correlation between appetite VAS score and cachexia stage.Conclusions: 1.The combination of Modified Chaihu Guizhi Decoction with megestrol and megestrol alone can improve the appetite and KPS score,alleviate the clinical symptoms of TCM and reduce IL-6 level in patients with both Yin and Yang deficiency cancer cachexia,but the combined group has better efficacy and is safe and reliable,which is worth promoting and applying.2.There is a positive moderate correlation between IL-6 level and cachexia staging(precachexia and cachexia stage),and the malignant stage has higher IL-6 level than the pre-malignant patients.There was a negative and weak correlation between the appetite VAS score and cachexia stage.
