Objective: By comparing the changes of soft and hard tissue profiles between adults and adolescents with Class Ⅱ division 1 and adolescents with high-angle extraction and correction, the importance of growth and development in orthodontic treatment is discussed, and clinical treatment is provided. Methods: Among the patients attending the Orthodontics Department of The Second Hospital of Dalian Medical University,28 cases of malocclusion and high angle patients with Class Ⅱ division 1 malocclusion were selected, including 13 cases in the adult group and 15 cases in the adolescent group. The treatment plan was to remove the four first bicuspids and the anterior teeth were retracted with strong anchorage.Lateral cephalometric were taken before and after treatment, and the cephalometric analysis was performed using Vceph5.0 software.Using SPSS26.0 software, the changes in the measured values of the two groups of patients before and after treatment were applied to the paired sample t-test,and the differences in the measured values of the two groups of patients were applied to the independent sample t-test.The changes in the soft and hard tissues of the two groups of patients before and after treatment and the aesthetic indicators of profile The correlation of using Pearson correlation analysis method.Results:(1)After treatment, the hard tissue measurement items in the adult group did not change significantly(P>0.05),while the mandibular body in the adolescent group increased significantly(P<0.001),the sagittal bone profile was improved, and the posterior to front height ratio increased(P<0.001).The vertical bone shape has been greatly improved. Both the adult group and the adolescent combined plane were rotated backward and downward(P<0.001),and the mandibular plane angle did not increase significantly(P>0.05).(2)The upper and lower anterior teeth of the two groups of patients were significantly upright recovery after treatment(P<0.001),and the overlay coverage was effectively improved. However,in the adult group, the upper and lower anterior teeth had a more upright axis and the lower anterior teeth had a lip orientation compensatory.(3)After treatment, the convexity of the soft tissue of the two groups of patients was improved to a certain extent, the nasolabial angle and the inclination of the upper lip were significantly increased, and the protrusion of the upper and lower lip was effectively improved. The convex angle of the soft tissues in the adolescent group was significantly reduced(P < 0.001),the inclination of the chin-labial groove was significantly increased(P<0.001),and the prominence of the profile was significantly reduced. The improvement in the profile of the adult group was not as good as that of the adolescent.(4)The changes in a number of hard tissue measurement items in the adolescent group are significantly related to changes in profile, such as ANB,NA-PA,PN Perpend,GoPog,etc.,while in the adult group, most of the indicators related to changes in profile are teeth.Sexual measurement values, the two groups showed large differencesConclusion: 1.In the adolescent group, treatment during the peak period of growth and development can express more bone effect and improve the bone type II to the greatest extent.2.The adult group has fewer changes in jaws, and mainly relies on tooth compensation to conceal the incoordination of jaw development.3.Reasonable use of Class Ⅱ intermaxillary traction for high-angle patients will not cause changes in the mandibular plane angle and face height.4.In patients with high angle of Class Ⅱ and 1 classification, the appearance of soft and hard tissues has been significantly improved in adolescents compared with adult correction.
