Objective:Research purpose: To analyze the serum IFN-γ(Th1 type cytokine),IL-33 a nd IL-4(Th2 type cytokine)expression levels and the levels of IFN-γ and IFN-γ in infants with lower respiratory tract infections caused by respiratory syncytial virus(RSV)Changes in the ratios of IL-33/IFN-γ and IL-4/IFN-γ;compare the c hanges of the three groups of cytokines and Th1/Th2 ratios in children with whe ezing and non-wheezing;explore the serum IL-33,IL-4,The role of IFN-γ expre ssion level and IFN-γ/IL-33,IFN-γ/IL-4 ratio in the pathogenesis of lower respira tory tract infections in infants and young children caused by RSV.Methods:A total of 57 infants and young children diagnosed as "lower respiratory tr act infection caused by RSV" who were hospitalized in the Respiratory Departme nt of Dalian Childrens Hospital from October 2019 to March 2020 and met the inclusion criteria were selected,and their age and gender were recorded.,Clinical and imaging findin gs,total serum IgE value(IU/mL),FeNO value(ppb).Among them,those with wheezing symptoms were classified as the wheezing group,a total of 28 cases,and those without wheezing symptoms were classified as the non-wheezing group,a total of 29 cases.At the same time,29 infants and young children who recei ved physical examination were the healthy control group.The sera of the three groups of children were detected by ELISA for IL-4,IL-33,and IFN-γ,and the concentration value(pg/mL)was recorded;the changes in the three groups of s erum IL-4,IL-33,and IFN-γ were observed,Calculate the ratios of IFN-γ/IL-3 3 and IFN-γ/IL-4 in each group and compare between groups;observe the clini cal manifestations and chest imaging characteristics of children after RSV infectio n,observe the wheezing group and the non-wheezing group Changes in serum to tal IgE and FeNO;and analyze the correlation between total IgE and FeNO leve ls in the wheezing group and serum IL-4,IL-33,IFN-γ.Resluts:General comparison: RSV caused by acute lower respiratory tract infections in infants and young children,the main clinical manifestations are cough(100%),wheezing(49.1%),chest imaging performance is mainly flaky blurred shadow 20 cases(35.1%),double lung texture enhancement 18 cases(31.6%).The difference in fe NO levels between the respite group and the non-breathing group caused by RSV was not statistically significant(P>0.01).The results of the total IgE comparison between the Wheezing Group and the Serum of the Non-Wheezing Group by RSV were statistically significant(P≤0.01).Comparison of cytokine measurements for each group:(1)The group of acute lower respiratory tract infections caused by RSV was statistically significant(P≤0.05)compared with the health control group,serum IFN-γ,IL-4,and IL-33.Tip If N-γ levels in the RSV infection group were lower than in the health control group,while the RSV infection groups IL-4 and IL-33 were higher than the health control group.(2)Compared with the health control group,serum IFN-γ,IL-4,and IL-33 were statistically significant(P≤0.05).The IFN-γ levels in the respite group were lower than in the health control group,while the IL-4 and IL-33 in the RSV wheezing group were higher than in the health control group(3)The non-wheezing group after RSV infection was statistically significant compared to the health control group,with serum IFN-γ,IL-4,and IL-33(P≤0.05).Suggest that IFN-γ levels in non-breathing groups were lower than in health control groups,while IL-4 and IL-33 levels were higher than in health control groups.(4)The if N-γ,IL-4,and IL-33 are statistically significant compared to the non-wheezing groups after RSV infection(P≤0.05).Tip The IFN-γ level of the wheezing group is lower than that of the non-breathing group,while the LEVEL of IL-4 and IL-33 is higher than that of the non-wheezing group.(5)Compared with the health control group,the RATIO of IFN-γ/IL-4,IFN-γ/IL-33 was statistically significant(P≤0.01).The RATIO OF IFN-γ/IL-4,IFN-γ/IL-33 for acute lower respiratory tract infection caused by RSV was lower than that of the health control group.(6)Compared to non-wheezing groups after RSV infection,The IFN-γ/IL-4,IFN-γ/IL-33 ratio is statistically significant(P≤0.05),suggesting that the if N-γ/IL-4,IFN-γ/IL-33 of the RSV after infection is lower than the non-breathing group;(7)Analysis of the correlation between the total serum IgE level and the level of IL-4,IL-33,IFN-γ(P>0.05)in the respite group after RSV infection.Tip The total serum levels of infants and young children with Wheezing due to RSV infection were not associated with IL-4,IL-33,IFN-γ.(8)The correlation between FeNO values and serum IL-4,IL-33,IFN-γ levels(P>0.05)in the respite group after RSV infection was analyzed,and suggested that the level of fe NO in infants and young children who gasped after RSV infection was not correlated with IL-4,IL-33,IFN-γ.Conclusion:1)IL-33 can be expressed in the serum of infants and young children who wheez after RSV infection.2)RSV infection,whether wheezing or not,can change the level of inflammatory factors in the body,and the expression level of cytokine Th2 increases.The levels of serum IL-4 and IL-33 in the RSV infection group were higher than those in the health control group.Comparing if N-γ/IL-4,IFN-γ/IL-33 with health control groups in RSV-induced infection group,it was suggested that the body had an immune imbalance of Th1/Th2 after RSV infection.3)The change levels of IL-4,IL-33,IFN-γ after RSV infection were significanter than those of non-wheezing groups,indicating that the imbalance of Th1/Th2 in the wheezing group after RSV infection was more severe,as shown by the levels of IL-4 and IL-33 in the Wheezing group after RSV infection The respite group increased,the IFN-γ decreased compared to the non-wheezing group,compared with the IFN-γ/IL-4,IFN-γ/IL-33 in the RSV-induced infection group,which was lower than the non-wheezing group,suggesting that the imbalance of Th1/Th2 in the wheezing group was significantly higher than that in the nonwheezing group. |
