Bullous pemphigoid(bullous pemphigoid,BP)is a chronic recurrent autoimmune disease with an increasing incidence year by year worldwide.BP is more common in patients over 60 years of age,often accompanied by a variety of complications and dysfunction,and the main clinical manifestations are erythema,blisters and bullae.Systemic glucocorticoid therapy as the main treatment for BP in clinical practice needs to be maintained for a long time according to the condition,which is easy to induce or aggravate serious adverse reactions such as infection,metabolic disorders and abnormal blood system,often with poor prognosis,which has a great impact on mental health of patients and the quality of life.This study retrospectively analyzed the data of 164 BP inpatients in the second Hospital of Jilin University from2013 to 2020,summarized and analyzed the clinical characteristics,laboratory indicators and complications of the BP patients,explored the related factors affecting the common BP complications,and provided theoretical basis for the treatment and prognosis of patients in the future.Objective:To summarize the clinical characteristics,laboratory indicators and treatment characteristics of 164 patients with bullous pemphigoid from 2013 to 2020 in the Department of Dermatology of the Second Hospital of Jilin University,analyze the clinical characteristics of BP,summarize the rules of experience and explore the related factors affecting the common BP complications,and provide reference for clinical diagnosis and treatment of BP and a certain basis for improving the quality of life of patients in the future.Methods:A retrospective analysis was used to select the medical records of 164 hospitalized BP patients in the Department of Dermatology of the Second Hospital of Jilin University from February 2013 to July 2020.The basic information,clinical features,misdiagnosis,complicated diseases,laboratory indicators,treatment methods,complication and efficacy of the patients were statistically analyzed.The related factors affecting BP complications were analyzed statistically.Result:1.Among the 164 inpatients,there were 99 males(60.37%)and 65 females(39.63%),with a male-to-female ratio of 1.5:1.The mean age of diagnosis was68.56±13.06 years old,and there were 121 patients(73.78%)over 60 years old.There was no statistical significance in age of onset among different genders(P> 0.05).The length of stay ranged from 6 days to 3 months,with a median of 20 days.2.In 164 patients,the primary lesions were mainly erythema,papules,blisters or bullae(50%),and trunk was involved first.The skin lesions were mainly in the trunk and limbs.There were 127 patients with varying degrees of pruritus with or without pain.3.Mucosal involvement was found in 38 patients(23.17%),and oral mucosal erosion was the first lesion in 2 patients.4.141 patients had at least one other disease(85.98%),and the major systemic diseases included cardiovascular system diseases in 84 cases,62 cases of diabetes,nervous system disease,40 cases,pneumonia in 40 cases,29 cases of the digestive system disease,other skin disease,22 cases,thyroid disease,20 cases,femoral head necrosis in 7 cases,6 patients with malignant tumor,4 cases of rheumatoid arthritis,respectively accounting for 59.57%,37.80%,28.37%,24.40%,20.57%,15.6%,14.18%,4.96%,3.66%,2.84%.Cerebrovascular diseases were the main diseases of the nervous system,accounting for 85%.there were 48 cases(29.27%)of infection of 64 BP patients during treatment.The complication of infection was related to the length of hospital stay,the maximum dose of glucocorticoids,the cumulative dose of corticosteroids,the duration of hormone dose reduction,mucosal involvement,pneumonia,diabetes,malignancy,the use of immunosuppressants,serum calcium reduction,hypoalbuminemia and the severity of disease.The difference was statistically significant(P<0.05),And diabetes,pneumonia and the use of immunosuppressants were independent risk factors for infection.The complication of diabetes in BP patients is related to the length of hospital stay,the maximum dose of glucocorticoid,the cumulative dose of glucocorticoid,dyslipidemia and abnormal renal function.The difference was statistically significant(P<0.05),and Dyslipidemia was an independent risk factor for diabetes mellitus.5.The severity of the disease in BP patients was related to hypoproteinemia,eosinophil elevation,D-dimer elevation,the difference was statistically significant(P<0.05).Whether the mucosal involvement of patients of BP was related to hypoproteinemia,the difference was statistically significant(P<0.05).The complication of platelet abnormalities was related to the length of hospital stay,the initial dose of glucocorticoid,the maximum dose of glucocorticoid,the cumulative dose of glucocorticoid,the duration of hormone dose reduction,mucosal involvement,pneumonia,diabetes,urinary microalbumin,hypoalbuminemia,and the use of cyclosporine.Difference was statistically significant(P<0.05).Diabetes,pneumonia and the use of cyclosporine were independent risk factors for platelet abnormalities in BP patients.6.Systemic glucocorticoid therapy was the main treatment,including 114 patients with hormone combined with immunosuppressant.The effective rate was 96.95%。The length of hospital stay,the maximum dose and cumulative dose of hormone in patients with combined use of hormones were higher than those with hormone alone.The initial dose and maximum applied dose of hormone in young and middle-aged patients were higher than those in elderly patients.The length of hospital stay,the initial dose,maximum applied dose and cumulative dose of hormone in patients with mucosal involvement were higher than those in patients without mucosal involvement.Conclusion:1.BP usually occurred in the elderly over 60 years old,and there was no significant difference in age between men and women,and it is often associated with one or more other systemic diseases,mainly cardiovascular diseases and nervous system diseases.Diseases of the nervous system are mainly cerebrovascular diseases.2.Most patients were admitted with increased neutrophils,eosinophils,D-dimer and decreased albumin levels.Combined hypoalbuminemia was one of the fatal risk factors of BP.the elevated levels of eosinophils and D dimers in peripheral blood,hypoalbuminemia were associated with the severity of the disease in BP patients(rash body surface area ratio),and hypoalbuminemia was associated with mucosal involvement in BP patients.3.BP patients in this study had 23 cases with platelet abnormalities,with an incidence of 14.02%,of which 19 had thrombocytopenia,with an incidence of 11.59.diabetes,pneumonia,and the use of cyclosporine were independent risk factors for thrombocytopenia in BP patients.4.Diabetes,pneumonia,use of immunosuppressants were independent risk factors for infection in BP patients,and dyslipidemia was a independent risk factors for BP complicated with diabetes.
