Objective:Our country is a high prevalence area of HBV,and in recent decades,the number of IBD patients showed a rapid upward trend.We conducted this study to clarify the relationship between HBV infection in IBD patients and the local general population,and to analyze the risk factors of HBV reactivation,and to compare the impact of the number of IMMS on it.Methods:The IBD patients who were hospitalized in China Japan Union Hospital of Jilin University from January 2013 to December 2020 were retrospectively analyzed.The IBD diagnosis of all patients should comply with the consensus on the diagnosis and treatment of inflammatory bowel disease(Beijing,2018)formulated by the inflammatory bowel disease group of Digestive Disease Branch of Chinese Medical Association.The clinical stage and type of IBD,treatment plan,HBV serological markers,HBV DNA and HBV reactivation before and after IBD treatment were collected.Results:From January 2013 to December 2020,1044 patients with IBD in China Japan Friendship Hospital of Jilin University were retrospectively analyzed,including 895 patients with UC and 149 patients with CD.The overall serological marker screening rate of IBD patients increased from 48.3% in 2013 to 98.6% in 2020;the HBsAg positive rate of IBD patients was 6.2%,including 6.1% of UC patients and 6.4% of CD patients;the proportion of HBsAg-/ HBc Ab + was 24.5%,including 25.3% of UC patients,The awareness rate and cognition rate of HBV infection in 1044 patients with IBD were 40.7% and 19.8% respectively.In this study,there was no significant difference in the positive rate of HBsAg and the ratio of HBsAg-/ HBc Ab + between IBD patients born in different years and local physical examination population.The awareness rate and cognition rate of HBV infection and related diagnosis and treatment in 1044 patients with IBD showed an upward trend with the change of years.Among them,the awareness rates of IBD patients with high,medium and low risk of HBV reactivation were 62.5%,45.0% and 9.1% respectively,and there were significant differences among them(P < 0.05).The cognition rate of high risk was 28.1%,and that of medium risk was 23.7%,which was significantly higher than that of low risk(1.5%),P < 0.05;Conclusions:(1)The HBsAg screening rate,the awareness rate and cognitive rate of anti HBV treatment of IBD inpatients increased with the change of the year from 2013 to 2020.(2)The proportion of HBsAg +/hbcbb+ and HBsAg-/hbcbb+ in IBD patients of different birth age was not significantly different from that of the local people.(3)In IBD patients,HBV reactivation occurred in HBsAg + / HBc Ab + patients.Patients with high and medium risk of HBV reactivation and patients with more than two kinds of immunosuppressants(IMMS)were more likely to have HBV reactivation.
