| Background:Human Papillomavirus(HPV)infection is the main factor causing cervical cancer and its early lesions.With the HPV genotype detection becoming a routine screening method and the popularization and application of HPV vaccines,cervical precancerous lesion have been found and treated early,which has reduced the incidence and mortality of cervical cancer,but the incidence of cervical cancer is still very high.Considering the popularization of HPV screening technology,the newly approval of the imported and domestic HPV vaccine in China and the prevalence of HPV distribution,which is meaningful for directing HPV vaccination and the development of a new generation vaccine.Objective:To investigate the clinical significance and the distribution of Human Papillomavirus(HPV)genotype among patients with Cervical Low-grade Squamous Intraepithelial Lesion(LSIL),High-grade Squamous Intraepithelial Lesion(HSIL)and Cervical Squamous Cell Carcinoma(SCC)in Taizhou.Methods:23 kinds of HPVgenotypes were detected by PCR combined with gene chip technology in histologically confirmed paraffifin-embedded specimens,which collected from patients with 489 cases of LSIL,356 cases of HSIL and 95 cases of SCC in Taizhou People’s Hospital since Jan 2019 to Dec 2019.Data of respondents was analyzed.Results:The total HPV infection rate was 83.83%(788/940),the high-risk HPV infection rate was 81.81%(769/940),the low-risk HPV infection rate was12.13%(114/940),and the top five high-risk genotypes were HPV16(35.64%),52(16.91%),58(13.94%),33(8.94%),and 18(7.98%).The main infection low-risk genotypes were HPV81(4.47%),6(2.87%),43(2.87%),and 42(2.02%).HPV positive rate of SCC group was significantly higher than those of LSIL and HSIL group(χ~2=13.083,P<0.001;χ~2=7.743,P=0.005);but there was on significant difference in HPV infection rate between LSIL and HSIL groups(χ~2=2.939,P=0.086).Single infection and double HPV infections were prevalent in SCC,LSIL,and HSIL.The positive rate of high-risk HPV in SCC group was significantly higher than in LSIL(χ~2=16.928,P<0.001)and HSIL group among all age groups(χ~2=8.928,P=0.003);The age groups with the highest HPV infection rate in all cervical lesion patients appeared in the≤29 years(84.88%)and≥60 years(85.11%),respectively.Conclusions:(1)The degree of cervical lesions is significantly associated with the genotype distribution of HPV,but not associated with the infection pattern.There were two peaks of HPV infection in cervical lesion patients,≤29 years old and≥60years old.The high-risk HPV positive rate was high in the all cervical lesions groups,but not related with age;With the increase of cervical lesions grades,the low-risk HPV positive rate gradualy increases.(2)High HPV infection rate and wide HPV genotypic spectrum in cervical lesions make the prevention of cervical cancer challenging.Given the distribution of non-vaccine-covered HPV53,HPV56,HPV81and HPV43 genotypes and the variation in HPV infection rates between different age groups,new HPV vaccine development programs based on regional genotypes and age infection features,could have a beneficial effect on the prevention of cervical cancer. |
