The Value Of VI-RADS Score In The Diagnosis Of Muscle-invasive Bladder Cancer

Purpose:To investigate the diagnostic efficacy of VI-RADS score in muscle-invasive bladder cancer.Methods:Patients with bladder cancer who underwent multiparameter MRI examination and underwent surgical treatment and obtained pathological results in our hospital from November 2019 to November 2020 were included in this study.The multiparameter MRI images of the patients,including T2 WI,DWI and DCE-MRI,were analyzed by two experienced radiologists without knowing the operative method and pathological results.The score and staging of the bladder tumor were evaluated by VI-RADS score.The accuracy of the results was compared with the postoperative histomathology.The ability of VI-RADS score in the diagnosis of muscle-invasive bladder cancer was evaluated using receiver operating characteristic curve and area under the curve.Results:All the patients were performed by the same surgeon,including transurethral resection of bladder tumor in 26 cases,radical cystectomy in26 cases,and partial cystectomy in 2 cases.Two radiologists observed multiparameter MRI images of each patient,and assigned scores using the VI-RADS scoring system,including 6 patients with 1 score,12 patients with 2 score,9 patients with 3 score,8 patients with 4 score,and19 patients with 5 score.Postoperative pathology proved NMIBC in 26 cases and MIBC in 28 cases.Using ROC curve for VI-RADS score diagnosis muscle-invasive bladder cancer were analyzed,and the effectiveness of the AUC value is 0.742,95% confidence interval(the confidence interval,CI)0.608 0.876,p < 0.001,the further analysis to the VI-RADS score > 3 points as the critical value,the diagnostic sensitivity and specificity of MIBC’s best,its sensitivity is 0.72,the specificity of0.69,positive likelihood ratio is 2.32,the negative likelihood ratio is 0.42,Youden index was 0.40.Conclusion:1、VI-RADS score has a good predictive value for evaluating MIBC。2、VI-RADS score has reference value for guiding the selection and judgment of MIBC clinical operation mode。...
