Objective:To explore the effect of aerobic exercise on the clinical symptoms of Parkinson’s disease and its possible mechanism.Methods:Fifteen patients with primary Parkinson’s disease who were admitted to the Parkinson’s Specialty Clinic of the Department of Neurology of the First Clinical Hospital of Jilin University were selected,and the original anti-Parkinson drug dose was kept constant for a period of 4 weeks(3 times a week,each 1 hour)aerobic exercise bike.At baseline,during exercise,after exercise,and after 2 weeks of follow-up,the scale was used to assess motor and non-motor functions at various time points,and transcranial Doppler ultrasound was used to observe changes in cerebral blood flow autoregulation.Lactate oxidase method was used to detect the level of peripheral blood lactate.Results:1.Compared with the baseline,the results of the 4-week aerobic exercise,the end of the exercise,and the follow-up after 2 weeks showed that the UPDRS-III score of PD patients showed a downward trend,and the difference was statistically significant(P=0.004).Among them,the bradykinesia subdomain score was significantly different from the baseline(P=0.001).In addition,after aerobic exercise,the subjects’ Mo CA score,PDQ-39 score and HAMD score were significantly different from baseline(P=0.015,P=0.040 and P=0.031).2.Compared with the baseline,during the 4-week aerobic exercise,the lactate level of PD patients showed a downward trend(P=0.297),although the lactate level during the follow-up 2 weeks after the exercise was higher than that at the end of the exercise,But there is still a downward trend compared with the baseline,and the difference is not statistically significant(P=0.071).3.Compared with the baseline,the evaluation results of cerebral blood flow autoregulation at the end of exercise and at the 2-week follow-up show that the difference in overall phase difference is statistically significant,F(2,20)=3.026,P=0.041.At the end of the exercise,the overall phase difference of the subjects increased by 19.350 from the baseline(95% confidence interval: 3.376-35.324,P=0.041).4.Spearman correlation analysis found that the level of lactate reduction in PD patients after aerobic exercise was positively correlated with the changes in the Hamilton Anxiety Scale and PDSS-2 Scale(P=0.034 and P=0.047).Conclusion:1.4-week aerobic exercise bike exercise is a simple,economical,feasible and effective exercise intervention method for PD patients.It can not only significantly improve the patient’s motor symptoms,but also improve some non-motor symptoms,including cognitive dysfunction,depression and quality of life.2.After aerobic exercise,the peripheral blood lactate level of PD patients decreased,suggesting that 4 weeks of aerobic exercise improved the energy metabolism of muscle tissue.3.The overall phase difference of PD patients increased after aerobic exercise,suggesting that 4 weeks of aerobic exercise improved the auto-regulation ability of cerebral blood flow.4.The decreased level of peripheral blood lactate is positively correlated with the improvement of anxiety and sleep quality,suggesting that the level of lactic acid may be involved in the improvement of anxiety and sleep quality in PD patients.
