| Objective:Through the epidemiological survey of sleep-wake disorders in Hebei province,to understand the current status of sleep-wake disorders in the elderly in Hebei Province and their medical visits,explore the influencing factors of insomnia symptoms in the elderly,and provide a scientific basis for improving the sleep of the elderly.Methods:This epidemiological survey adopts stratified cluster random sampling method to carry out the research,and the research objects are the permanent residents of Hebei Province 60 years old and above.Use the general information questionnaire to collect the gender,age,place of residence,education level,marriage,and monthly household income of the permanent population;use the life history questionnaire to investigate the gas poisoning experience and memory loss caused by head trauma;use the behavior and lifestyle questionnaire to collect the elderly Eating pickled food,eating fried food,drinking,smoking;using the physical condition questionnaire to collect cerebrovascular disease,hypertension,and diabetes;the screening tools for sleep arousal disorder use the Athens Insomnia Scale,Sleep Apnea-Berlin Questionnaire,RLS Questionnaire and other scales,using simple sleep belief and attitude scales to assess beliefs about poor sleep.Analyze the distribution of various sleep-wake disorders in different populations,and apply multi-factor logistic regression analysis to explore the relationship between insomnia symptoms and influencing factors in the elderly over60 years old in Hebei Province.Results:1.A total of 7,302 people over 60 years old completed the survey of sleep-wake disorders in Hebei Province,including 3,298 males and 4004 females.The detection rate of insomnia symptoms was 21.9%,suspected OSAS was 24.5%,suspected RBD was 9.0%,suspected RLS was 2.6%,and suspected narcolepsy was 0.3%.2.Insomnia symptoms(X~2=190.713,P<0.05),suspected OSAS(X~2=9.488,P<0.05),suspected RBD(X~2=20.389,P<0.05),suspected RLS(X~2=10.433,P<0.05),the female detection rate is higher than that in men,suspicious narcolepsy(X~2=1.020,P>0.05)is not significant.Insomnia symptoms(X~2=86.329,P<0.05),suspected RBD(X~2=38.172,P<0.05),suspected RLS(X~2=19.258,P<0.05)rural detection rate is higher than the city,suspicious OSAS(X~2=1.267,P>0.05)and the urban and rural differences of suspected homogenesis(X~2=1.002,P>0.05)are not significant.Insomnia(X~2=14.702,P<0.05),suspicious OSAS(X~2=4166,P<0.05)age difference is statistically significant,suspicious RBD(X~2=3.655,P>0.05),suspected RLS(X~2=5.969,P>0.05),there is no significant difference in the differences of suspicious homogeneous sleeping(X~2=0.887,P>0.05).3.Suspected OSAS(X~2=119.642,P<0.05),suspected RBD(X~2=121.474,P<0.05),suspected RLS(X~2=31.379,P<0.05),suspected narcolepsy(X~2=5.570,P=0.018).The detection rate is higher than that of the non-insomnia.4.Multi-factors of sensitive symptoms of senior people discovery:Risk factors for insomnia symptoms of the elderly include women(OR=2.013,95%CI=1.711-2.369),head trauma(OR=2.012,95%CI=1.014-3.990),diabetes(OR=1.248,95%CI=1.038-1.501),high blood pressure(OR=1.306,95%CI=1.146-1.489),cerebrovascular disease(OR=1.719,95%CI=1.440-2.052),Poor sleep attitude and belief(OR=0.949,95%CI=0.944-0.955),negative response mode(OR=1.023,95%CI=1.014-1.032),negative life event(OR=1.031,95%CI=1.020-1.041)The protective factors of the insomnia symptoms of the elderly include residential cities(OR=0.631,95%CI=0.531-0.750),family monthly revenue is 1000-3,000 yuan(OR=0.782,95%CI=0.670-0.913).5.Among the elderly with insomnia symptoms,the rate of consultation was 5.4%,and the proportion of those taking sleep-aid drugs was 14.7%.Conclusion:The survey of the epidemic of sleep awakening obstacles in Hebei Province has found that the detection rate of insomnia symptoms in the elderly is 21.9%.Women,head trauma,diabetes,hypertension,cerebrovascular disease,bad sleep attitude and belief,negative response,negative life incident is the risk factor for insomnia for the elderly.OSAS,RBD,RLS and psychosis of the symptoms of the old insomnia.Older women’s OSAS,RBD and RLS have higher detection rates than men.RBD,RLS detection rate rural is higher than the city.As the age increases,the detection rate of OSAS gradually decreases.The problem of insomnia in the elderly is low due to insomnia,and the drug rate is higher than the treatment rate. |
