Effects Of Repetitive Transcranial Magnetic Stimulation On Serum Hs-CRP And IL-6 In Patients With Post-stroke Depression

Objective: To investigate whether the possible mechanism of repetitive transcranial magnetic stimulation(rTMS)to improve depressive symptoms in patients with post-stroke depression(PSD)is related to its regulation of the expression levels of serum inflammatory factors high-sensitivity C-reactive protein(hs-CRP)and interleukin-6(IL-6)in patients,and to provide scientific basis for rTMS’ treatment of depression in PSD patients and promoting neural functional in clinical practice.Methods: A total of 90 patients with acute ischemic post-stroke depression admitted to the Affiliated Hospital of Yanbian University from August 2020 to February 2021 were selected and divided into control group、drug group and magnetic stimulation group by random digital table method,with 30 cases in each group.The control group was given conventional therapy of antithrombotic,improving circulation and nourishing nerves;escitalopram oxalate was added to the drug group on the basis of conventional therapy;the magnetic stimulation group added transcranial magnetic stimulation on the basis of conventional drugs;then observe the changes of various depression scores such as NIHSS and HAMD scores,as well as the expression levels of hs-CRP and IL-6 in serum before and after treatment in the three groups.Results:(1)Pre-treatment NIHSS scores among the three groups have no statistical significance.After treatment,NIHSS scores decreased in all groups.The stroke scale score in the magnetic stimulation group was lower than that in the control group and drug group,and the difference was statistically significant(P < 0.05).The stroke scale scores of the three groups decreased compared with those before treatment,especially in the drug and magnetic stimulation group,and the differences among the three groups before and after treatment were statistically significant(P < 0.05).(2)There was no statistical difference in HAMD scores before treatment among the three groups(P > 0.05).After treatment,HAMD scores decreased in all groups.HAMD score in magnetic stimulation group was significantly lower than that in control group and drug group.HAMD score in drug group was lower than that in control group,which had statistical difference(P < 0.05).After treatment,the scores of depression scale in drug group and magnetic stimulation group were lower than those before treatment,and the differences were statistically significant(P < 0.05).(3)There was no significant difference in hs-CRP level among the three groups before treatment.After treatment,the content of hs-CRP in magnetic stimulation group was lower than that before treatment,and significantly lower than that in the treated control group,and the difference had statistical significance(P < 0.05).(4)There was no significant difference in IL-6 levels among the three groups before treatment.After treatment,IL-6 levels in the magnetic stimulation group were lower than those in the control group after treatment,and the difference had statistical significance(P < 0.05);IL-6 levels in the drug group and magnetic stimulation group after treatment were significantly lower than those before treatment,and the differences had statistical significance(P < 0.05).(5)Compared with the reduction rate of depression scale,4 cases in the control group,16 cases in the drug group and 25 cases in the magnetic stimulation group were effective after treatment.The total effective rate of the magnetic stimulation group was higher than that of the control group and the drug group,and the total effective rate of the drug group was higher than that of the control group.The differences were statistically significant(P < 0.05).Conclusion:(1)rTMS can improve the depressive symptoms of patients with PSD,and promote the recovery of neurological function,and its effect is better than drug treatment.(2)The mechanism of action of rTMS treatment may be related to reducing the expression levels of hs-CRP and IL-6 in the serum of PSD patients,thereby reducing the inflammatory response in vivo.
