Objectives: through the implementation of health education intervention based on the health management model for stroke patients,to explore the impact on stroke patients’ self-management behavior,perception control,activities of daily living and biochemical indicators,and to clarify the effective intervention effects on stroke patients,and Provide a scientific basis for the hospital to carry out continued nursing.Methods: after proposing the hypothesis of this research,in order to verify the hypothesis,we used our own before-and-after experimental research method to conveniently select 80 stroke patients who were hospitalized in a tertiary general hospital in Jilin Province based on the inclusion and exclusion criteria,and implemented them A 3-month health education intervention based on a health management model,using general demographic data and disease-related data questionnaires,stroke self-management behavior evaluation scales,and multiple dimensions before intervention,1 month of intervention,and 3 months of intervention,respectively Health Control Scale(MHLC-C),Modified Pap Index(MBI)and Physiological and Biochemical Index Questionnaire to evaluate the effect of health education intervention based on the health management model in the management of stroke patients,using SPSS26.0 statistics The software performs statistical analysis of the data.Results:(1)General conditions of stroke patientsIn this study,patients were between 42 and 79 years old,with the majority of elderly patients accounting for 65%;male patients accounted for a larger proportion than female patients,at 73.8%and 26.2%,respectively;Han people accounted for more than minority The proportion of patients is slightly larger,52.5% and 47.5% respectively;the occupation is mostly retired/other patients,accounting for 27.5%;63.8% of patients have a family history of stroke;55.0% of patients have a family monthly income of less than 3,000 yuan;Yes Medical insurance patients accounted for95.0%;61.2% of patients smoked;43.8% of patients drank alcohol;32.5% of patients rarely took physical exercise;21.2% of patients were significantly overweight or obese.(2)Comparison of self-management behaviors of stroke patientsThe scores of pre-intervention,1 month and 3 months of intervention were(152.38±16.03),(163.52±18.20)and(181.42±16.50)respectively.The self-management behavior scores of stroke patients after the intervention were higher than those before the intervention.The effect of 3 months was better than that of 1 month of intervention,and the difference was statistically significant(P<0.01).(3)Comparison of Perceptual Control in Patients with StrokeThe International Health Locus of Control and Powerful Others Health Locus of Control scores of the intervention for 1 month and the intervention for 3 months were significantly higher than before the intervention,while the Chance Health Locus of Control score was lower than before the intervention,the difference was statistically significant(P<0.01);the intervention for 3 months The scores of Health Locus of Control and Powerful Others Health Locus of Control were significantly higher than the one-month intervention,while the Chance Health Locus of Control score was lower than the one-month intervention,indicating that the effect of intervention for 3 months was better than that of intervention for 1 month,and the difference was statistically significant(P<0.01).(4)Comparison of activities of daily living in stroke patientsThe scores before intervention,1 month of intervention and 3 months of intervention were(59.96±11.34),(63.31±10.25)and(70.45±11.04)points,respectively.The scores of activities of daily living after the intervention were higher than those before the intervention,and the intervention The 3-month effect was better than one-month intervention,and the difference was statistically significant(P<0.01).(5)Comparison of biochemical indicatorsThe index values of TC,TG,LDL-C and Hcy at 1 month and 3 months after the intervention were significantly lower than before the intervention,the difference was statistically significant(P<0.05),while HDL-C was significantly higher Before intervention,the difference was statistically significant(P<0.01).The FPG and Hb Alc indicators decreased after the intervention 1 month and 3months after the intervention,but the difference was not significant(P>0.05).Conclusion: Health education intervention based on the health management model can improve the self-management behavior level and perception control ability of stroke patients,improve the patient’s disease-related physiological and biochemical indicators,and ultimately improve the activities of daily living of stroke patients.
