OBJECTIVE: A comparative study was conducted on the effects of carbapenems before and after clinical pharmacists participated in the application management of carbapenems in respiratory department of a hospital.To improve the rationality of the use of carbapenems and reduce the production of drug-resistant bacteria by participating in the formulation of treatment plan,review of medical orders,pharmaceutical ward round,case discussion,monitoring of adverse drug reactions,evaluation of drug efficacy and investigation of patients’ prognosis.METHOD: A total of 144 cases of carbapenem antibiotics treated in the Department of Respiratory Medicine of the hospital from January 2019 to June 2020 were selected.Among them,76 cases of clinical pharmacists who did not participate in the management were taken as the control group,and the routine pharmaceutical management was implemented.The intervention group included 68 cases with clinical pharmacists participating in management.Retrospective analysis was performed in the control group and prospective analysis was performed in the intervention group.The patients’ age,gender,clinical diagnosis,carbapenem drug types,drug use amount,medication course,hospitalization date,etiological examination results,medication indications,drug combination,usage and dosage,rationality evaluation,etc.,were compared and analyzed.RESULT: 1.Comparison of the results between the control group and the intervention group: the number of days in hospital,the cost of carbapenems and the average practical days of carbapenems in the intervention group were all lower than those in the control group,and the differences were statistically significant(P<0.05);The reasonable application of carbapenems in the intervention group was higher than that in the control group,and the difference was statistically significant(P<0.05).2.The investigation on the use of carbapenems antibiotics in the control group and the intervention group showed that the rational use rate of carbapenems antibiotics after intervention was higher than that in the control group,such as the indication of carbapenems antibiotics application,combination of drug use,medication course,and rate of microbial specimen examination,and the difference was statistically significant(P<0.05).3.Through clinical pharmaceutical care management of carbapenems antibacterial drug application,clinical pharmacists showed that the frequency of carbapenems antibacterial drug use decreased significantly,the rate of microbial specimen examination increased,the rationality of clinical drug use increased,the rate of microbial specimen examination increased significantly,and clinical drug use tended to be reasonable.CONCLUSION: The intervention and management of clinical pharmacists can significantly improve the rational level of clinical application of carbapenems,and effectively reduce the frequency of use of carbapenems antibiotics,the days of hospitalization,the cost and days of use of carbapenems antibiotics.Clinical pharmacists should improve information management,strengthen the supervision and management of antimicrobial drugs,deepen the clinical pharmacy work,and cooperate with medical staff sincerely,integrate clinical pharmacy into the treatment work,and promote the rational use of antimicrobial drugs with the knowledge of clinical pharmacy.
