| Objective:Rural areas are the focus of poverty alleviation work.Poverty caused by disease and returning to poverty due to disease are one of the main factors causing poverty,and also the main problems to be tackled in the battle against poverty.in the post-poverty era,China has completed out of poverty after an all-round victory in this task,however,under the current Chinese standards,the implementation eliminating absolute poverty does not mean that our country to solve the poverty problem,and indicates the rural poverty in China will enter into the next to the transformation of secondary poverty for the characteristics of the new stage of the relative poverty and multidimensional poverty will becoming a main contradiction of poverty alleviation work,how to implement sustainable poverty reduction work is still faced with the challenge of different dimensions.The top-level design and institutional arrangement of the targeted health poverty alleviation mechanism should focus on the possibility of individuals,groups or organizations falling into a low level of welfare after being damaged by health risk factors,that is,the vulnerability of health poverty.Social capital reflects the social living conditions of rural residents,some researches and practices show that it can improve the health status of rural residents,improve the ability to lower the health risk,and reduce the economic burden of diseases.From the perspective of social capital,the influence of social capital on the poverty vulnerability to health of rural residents is to supplement and perfect the theory of health poverty,can improve the efficiency of health poverty alleviation work,enhance the effectiveness of health policy for poverty alleviation and precision,to improve poverty reduction after age health poverty alleviation policy to provide inspiration and new perspective.Therefore,this study explores the influence mechanism of rural residents’ social capital on health poverty vulnerability,and provides a scientific basis for the formulation of forward-looking health poverty alleviation policies.Methods:Based on the social capital theory,health ecology dynamic theory,the cycle of poverty,poverty and anti-poverty theory,using China health and retirement survey 2018(CHARLS)database,on the basis of literature review clear rural social capital and health poverty vulnerability of connotation and denotation,build the evaluation index system of rural social capital,selection of China’s poverty line in 2018,3535 yuan/year,the world bank to develop the international poverty line of $2 per person per day,and "high income countries" high poverty line of $3.2 per day as the poverty line,Using a three stage feasible generalized least squares method(FGLS)of three kinds of rural health poverty vulnerability under the poverty line to measure,and carries on the descriptive statistics research respectively,through the principal component analysis(pca)integrated rural residents the index of different dimensions of social capital,construct Tobit regression model,study the inner difference of health poverty vulnerability and rural residents’ social capital’s impact on the health poverty vulnerability mechanism,using the quantile regression model to further study the social capital in different under the poverty line of different points on the effects of poverty vulnerability to rural residents health.Results:(1)The construction of rural residents’ social capital index system:Based on the existing data and literature analysis,the evaluation index system of rural residents’ social capital was constructed with 19 indicators including 4 dimensions(social trust,reciprocity,social participation,economic support and life care).The mean value,standard deviation,coefficient of variation and comprehensive coefficient of each index score can be obtained by calculation.(2)Health poverty vulnerability index of rural residents:A total of 11,342 rural residents were selected from the CHARLS database in this paper.When the poverty line was 3535 yuan per year,the health poverty vulnerability of rural residents in China was 0.431,that is,the probability of rural residents falling into poverty in 2018 was 0.431,less than 50%.When the poverty line is $2/day,the health poverty vulnerability of rural residents in China is as high as 0.504,more than 50%.When measured by the high poverty line,the poverty vulnerability of China’s rural residents reached 61.54%with the poverty line of $3.2/day.(3)rural social capital’s impact on health poverty vulnerability:under different poverty line the influence factors of independent variables have significant differences,on the whole,self-reported health,mental state,the individual social participation,individual economic support in three types of social capital and community under the poverty line will be poverty vulnerability index to the health of rural residents have a significant positive influence,the individual social capital of three rural residents under the poverty line health poverty vulnerability index have a significant negative effect.The results of quantile regression showed that the individual life care has been the main influencing factor of the low level,the individual social participation has been the most obvious influencing factor of the middle and high level of rural residents’ health poverty vulnerability,and the different vulnerability levels of individual economic support and individual life care have been affected by the poverty line.Community social capital has a significant influence on the middle and high scores,the influence is more obvious on the high scores.This indicates that community social capital is an important factor in the middle and high scores of rural residents’vulnerability to health poverty.Conclusions:(1)The fragile state of health poverty of rural residents needs to be improved,and there are significant differences in the vulnerability of health poverty with different self-rated health levels.(2)The four dimensions of rural residents’ social capital have different influences on the vulnerability of health poverty.(3)There are differences in the impact of social capital on the vulnerability of health poverty under different subscales.(4)Focusing on details and accurately establishing a long-term mechanism to solve relative poverty to consolidate the achievements of poverty alleviation;Strengthen medical and health security for vulnerable groups with high health poverty and improve their health level;Improve the quality of life of rural residents,pay attention to the mental health of the whole people;Give full play to the positive impact of social capital on reducing the vulnerability of rural residents to health poverty in the post-poverty era;We will continue to combine poverty alleviation with intellectual and moral support,and establish a social support network. |
