Effects Of Triple Manipulation And Acupuncture On Diffusion Tensor Of Cervical Muscle Mass In Patients With Neck Type Cervical Spondylosis

Objective:To verify the correlation between the clinical efficacy of triple manipulation and acupuncture on the patients with neck type cervical spondylosis and the variation of MRI diffusion tensor imaging parameters of cervical muscle group,to explore the significance and feasibility of MRI-DTI in the diagnosis and treatment of neck type cervical spondylosis,and to provide a new scientific method and effective method for the diagnosis and treatment of neck type cervical spondylosis.Methods:This study collected 30 patients with neck type cervical spondylosis treated in our hospital from December 2017 to December 2018.which were randomly divided into the experimental group and the control group with 15 patients in each group.The experimental group was treated with triple manipulation,while the control group was treated with acupuncture.The two groups were treated once every 2 days,30-40 min each time,three times a week,two weeks as a course of treatment,after the end of the course of treatment for efficacy evaluation.The two groups of patients were compared before and after treatment and 3.0t conventional magnetic resonance(MRI)plain scan and magnetic resonance diffusion tensor imaging(mri-dti)scan before and after treatment,respectively.The Region of Interest(ROI)of the corresponding muscle group was selected as the data center to measure the parameter values.Generally,the posterior lower margin level of the fourth vertebra of the cervical spine was selected for parameter measurement,and the FA value and ADC value of the corresponding abdominal muscle ROI of the anterior and posterior cervical vertebrae of the treatment were measured respectively.The data were analyzed and processed by IBM SPSS20.0,and the difference was statistically significant(P<0.05)and statistically significant(P<0.01).Result:Before and after treatment,the clinical efficacy of the triple manipulation group and the conventional acupuncture group were compared respectively,and the two groups had statistical significance(P = 0.045<0.05).The total effective rate of the triple manipulation group was higher than that of the acupuncture group,and the clinical efficacy of the triple manipulation group was better for the neck type cervical spondylosis.The FA values of the left clip-head muscle,cervical clip-neck muscle and sternocleidomastoid muscle decreased after the triple manipulation treatment.The FA values were P=0.04(P<0.05),P=0.02(P<0.05),and P=0.003(P<0.01),respectively.The changes of FA values were statistically significant.The FA values of the right craniocapsid muscle,bilateral cephalic hemispinous muscle,right cervical clamp muscle,bilateral scapularis levator muscle and right sternocleidomastoid muscle were compared by P > 0.05,which was not statistically significant.The ADC values of the right cervical clip muscle and the left and right sternocleidomastoid muscle were decreased,and the ADC values were P=0.017(P < 0.05),P=0.004(P < 0.01),and P=0.008(P < 0.01),respectively.The changes of ADC values were statistically significant.The ADC values of bilateral head clippers,head hemispines,scapularis levator and left neck clippers were compared by P > 0.05,which was not statistically significant.After acupuncture treatment,the FA values of the head splinter muscle on the left and right sides and the left cervical splinter muscle were decreased.The FA values were P=0.04(P < 0.05),P=0.01(P < 0.05),and P=0.005(P < 0.01),respectively.The FA value changes were statistically significant.However,the FA values of bilateral hemispinous head muscles,levator scapulae muscles,sternocleidomastoid muscles and right cervical clamp muscles were compared by P > 0.05,which was not statistically significant.The ADC values of the cervical clamps on the left and right sides were decreased,and the ADC values were P=0.009(P < 0.01)and P=0.02(P< 0.05),respectively.The changes of ADC values were statistically significant.However,the ADC values of bilateral head clips,head hemispines,scapularis levator and sternocleidomastoid were compared by P > 0.05,which was not statistically significant.The changes of FA values and ADC values were not symmetrically decreased or increased,indicating that the strain of cervical muscle mass in patients with neck type cervical spondylosis was not symmetrically or regularly observed,which was related to human life habits,posture and other factors.The improvement of the strain state of cervical flexor muscle mass can affect the change of MRI-DTI and play an important role in the treatment of pain and other discomfort symptoms.Conclusion:(1)With the improvement of the symptoms of neck type cervical spondylosis patients in both groups,the FA value and ADC value of each parameter of MRI-DTI of cervical flexural muscle mass showed a decreasing trend,which proved that there was a certain correlation between the clinical efficacy of triple manipulation and acupuncture on neck type cervical spondylosis and the MRI-DTI of cervical muscle mass.(2)Triple manipulation and acupuncture have a definite clinical effect on cervical spondylosis patients.Triple manipulation can improve the symptoms of neck type cervical spondylosis patients more obviously without any other adverse reactions,which has clinical guiding significance.(3)MRI-DTI technique is safe and effective for the clinical prediction of cervical spondylosis.
