| Objective:To explore the clinical effect of modified Baduanjin exercise on the early rehabilitation for lumbar disc herniation(LDH)patients after Percutaneous endoscopic lumbar discectomy(PELD).With the advantage of traditional Chinese qi gong,it can promote the early recovery after surgery and provide new thoughts for the rehabilitation of lumbar diseases with traditional Chinese qi gong.Methods:This project selected 60 patients with a clear diagnosis of LDH from June 2020 to November 2020 in Jiangsu Province Hospital of Chinese Medicine who have successfully undergone PELD surgery and met the criteria as the research objects.The surgeons are chief physicians.According to the patients admission date,the observed cases were divided into the Baduanjin group and the control group with 30 cases each through a random number table.The Baduanjin group was supplemented with modified Baduanjin exercise rehabilitation prescription(Exercise twice a day,once in the morning and once in the evening,six days a week,one consecutive month as a course,totally lasting for three courses)while the control group received conventional rehabilitation treatment.Compare the evaluation of waist and leg pain(VAS),lumbar function JOA score,and lumbosacral multifidus surface electromyography measurement before rehabilitation and one month after rehabilitation,and three months after rehabilitation between the two groups to determine the clinical rehabilitation efficacy and multifidus function.All data were processed with SPSS25.0 statistical software,and the overall efficacy of the two groups of patients was evaluated three months after recovery.Results:1.Compare the baseline data of the two groups such as gender ratio,average age,height,weight,course of disease,surgical segment,etc.The basic demographic characteristics of the two groups were not significantly different(P>0.05),which were comparable.2.VAS score comparison:Comparison within the two groups:There was statistical difference in the two groups before and one month after recovery,before and three months after recovery,one month after recovery and three months after recovery(P<0.05).Comparison between the two groups:The comparison between the groups before and one month after the recovery(P>0.05),there was no statistical significance;the comparison between the groups after the recovery three months(P<0.05),the difference was statistically significant.The VAS scores of the two groups decreased to varying degrees.One month after recovery,patients in the two groups had similar pain relief.After three-month-recovery,there was a significant downtrend in the Baduanjin group,and patients in the Baduanjin group had better pain relief effects than those the control group.3.JOA score comparison:Comparison within the two groups:The difference was statistically significant before and after recovery,one month before and after recovery,1 month after recovery and 3 months after recovery(P<0.05).Comparison between the two groups:The comparison between the groups before recovery was not statistically significant(P>0.05);the comparison between the groups one month after recovery and 3 months after recovery(P<0.05),the difference was statistically significant.The JOA scores in the two groups of patients were higher than before recovery,and the increase in the Baduanjin group was greater than that of the control group,indicating that patients in the Baduanjin group had better effect on lumbar function rehabilitation than those in the control group.4.The effect of the multifidus muscle on the affected side of the lumbosacral area:Comparison of AEMG,MPF,and MF values within the two groups:there was statistical difference respectively among the two groups of patients before recovery,one month after recovery,and three months after recovery,before recovery and one month after recovery,before and three months after recovery,one month after recovery and three months after recovery(P<0.05).Comparison between the two groups of patients:As for the comparison of AEMG,MPF,and MF values between the two groups:there was no statistical significance before and one month after recovery(P>0.05).However,the difference was statistically significant between groups three months after recovery(P<0.05).The values of AEMG,MPF,and MF of the two groups increased to varying degrees.One month after recovery,the values of AEMG,MPF,and MF of the two groups increased similarly.Three months after recovery,the values of AEMG,MPF,and MF in the Baduanjin group increased more dramatically than those in the control group,which implied that the Baduanjin group has better effect on improving the strength and fatigue of the multifidus muscle than the control group.5.The effect of the multifidus muscle on the healthy side of the lumbosacral region:Comparison of AEMG,MPF,and MF values within the two groups;In terms of MPF and MF values there was statistical difference in each group before and one month after recovery,before and three months after recovery,one month after recovery and three months after recovery,as well as AEMG value in the Baduanjin group(P<0.05).AEMG values,When it came to the control group,the statistical difference was significant in terms of AEMG value before and one month after recovery,along with before and three months after recovery(P<0.05).The difference was not statistically significant between one month after recovery and three months after recovery(P>0.05).Comparison of AEMG,MPF,MF values between the two groups:There was no statistical difference before and one month after recovery(P<0.05).However,the statistical difference is significant between groups three months after recovery.The values of AEMG,MPF,and MF in the two groups increased to varying degrees.One month after recovery,the values of AEMG,MPF,and MF in the two groups increased similarly.Three months after recovery,the values of AEMG,MPF,and MF in the Baduanjin group increased dramatically than that in the control group,which implied that patients in the Baduanjin group had better improvement in terms of the strength and fatigue of multifidus muscle than the control group.6.Overall curative effect comparison:The excellent and good rate of the Baduanjin group is 90%while that of the control group is 80%(P<0.05),which is statistically significant.The results show that the Baduanjin group is better than the control group in terms of efficacy.Conclusion:Modified Baduanjin combined with conventional rehabilitation exercises can improve the rehabilitation effect of patients after PELD.Long-term adherence to Baduanjin exercise can effectively relieve the muscle strength and fatigue of the lumbosacral muscles for patients with LDH after PELD,and improve the lumbosacral pain and lumbar spine function,which is a safe,effective and beneficial way of daily physical exercise,and it is worthy of popularization and application in the rehabilitation of LDH patients after PELD. |
