| Purpose:This study collected TCM Constitution Types and general information of thyroid nodules through questionnaire survey,analyzed TCM Constitution distribution rules of thyroid nodules patients,studied the correlation between general information,thyroid function,elastic B-ultrasound and TCM Constitution of different thyroid nodules patients,explored the significance of TCM Constitution in the prevention and treatment of thyroid nodules from the perspective of "constitution relating to disease" of thyroid nodules.Methods:In this study,200 patients with thyroid nodules from May 1,2020 to December 31,2020 in the Department of endocrinology of Affiliated Hospital of Nanjing University of Traditional Chinese Medicine were selected.The patients were asked to fill in the clinical constitution questionnaire subjectively.The constitution questionnaire was formulated according to the constitution classification and judgment form of traditional Chinese Medicine issued by the China Association for Traditional Chinese Medicine.Combined with the inquiry of Professor Wang Xu on the patient’s condition the standard correction and the judgment were made,The basic information of patients(Age,gender,height,weight,general life and diet,etc),thyroid elasticity B-ultrasound and thyroid function examination data were collected.SPSS 26.0 statistical analysis software was used to describe the above data and explore the correlation between TCM constitution classification of patients with thyroid nodule.Finally,according to the research results,summarize and refine the reference significance of TCM Constitution for the prevention and treatment of thyroid nodules.Result:(1)The proportion of female patients with thyroid nodules in the endocrinology clinic of Affiliated Hospital of Nanjing University of Traditional Chinese Medicine is the majority,mostly in young and middle-aged people less than 50 years old.BMI mostly belongs to the normal range,there are more work pressure and emotional fluctuations,there are no bad habits of smoking and drinking,mental workers are the majority,and there are more unbalanced diet taste.(2)In this study,74.50%of patients with thyroid nodules were diagnosed as benign nodules,and 25.50%were suspected of malignant tendency.The average level of thyroid function in patients is in normal range.Thyroid function has the situation of thyroid autoantibody titer rising.(3)Most of the patients with thyroid nodule have non peaceful constitution,66%of them have two or more types of constitution.The order of constitution distribution from more to less is Yang deficiency>qi stagnation>damp heat>Qi deficiency>Yin deficiency=phlegm dampness>blood stasis>special nature.Phlegm dampness and dampness heat are easy to merge,blood stasis and qi stagnation and Qi deficiency are easy to merge,Yang deficiency and phlegm dampness,dampness heat,Qi deficiency,qi stagnation and blood stasis may merge.(4)The levels of TSH,FT3 and FT4 were different in different constitution.The results of the two comparisons showed that:The level of FT3 in patients with mild constitution was higher than that in patients with Yang deficiency and Qi deficiency;the level of FT3 in patients with damp heat constitution was higher than that in patients with Qi deficiency and yang deficiency;the level of FT3 in patients with qi depression constitution was higher than that in patients with Qi deficiency and yang deficiency.The level of FT4 in patients with damp heat syndrome was higher than that in patients with Qi deficiency syndrome.The TSH level of patients with Yang deficiency,Qi deficiency and phlegm dampness was higher than that of patients with damp heat.(5)The distribution of suspicious benign and malignant nodules of negative constitution type and positive constitution type was statistically different,and suspicious malignant nodules were more common in patients with negative constitution type.(6)Age,gender,emotional fluctuation,BMI,work pressure,diet taste and so on may have an impact on the physical type of patients with thyroid nodules.Age is the risk factor of Yang deficiency;BMI is the risk factor of qi stagnation;gender is female,emotional fluctuations,work pressure is the risk factor of biased constitution;like to eat sour taste is the risk factor of Yang deficiency;sadness is the risk factor of qi stagnation;happiness is the protective factor of qi stagnation.Conclusion:The distribution of TCM Constitution in thyroid nodules patients is mainly biased constitution,with Yang deficiency,qi stagnation,and mixed constitution.General information such as age,gender,emotion,work pressure and diet taste may have an impact on the type of constitution.Suspicious malignant nodules were more common in patients with negative constitution type,TSH level was higher than that in patients with positive constitution type,and thyroid hormone(FT3,FT4)level was lower.The above conclusions can provide reference for the clinical prevention and treatment of thyroid nodules. |
