BackgroundChronic obstructive pulmonary disease(COPD)is a preventable and treatable disease characterized by persistent respiratory symptoms and airflow limitation,which results in heavy burden to the family and society.Pulmonary function test provides objective measures to evaluate airflow limitation,but it fails to reflect the condition of patient completely and evaluate clinical curative effect sensitively in a short term.In order to provide accurate treatment,it is necessary to comprehensively evaluate the patient’s condition.At present,there are many evaluation systems for COPD,such as CAT,mMRC,BDI,SGRQ,CCQ et,al.ObjectiveTo comprehensively evaluate the condition of patients through different evaluation systems and explore the practicality and clinical significance of different evaluation systems in the process of clinical diagnosis and treatment;to investigate the efficacy of different inhaled medicines on various aspects of patients through different evaluation systems.MethodsPatients with acute exacerbation of COPD,who were admitted to the Department of Respiratory Medicine,The Third Affiliated Hospital of Southern Medical University,from November 2020 to January 2021 were collected.1.Patients were performed the examinations of CAT,mMRC,BDI,CCQ,SGRQ,BODE and HDRS.The correlation between the different evaluation systems was analyzed;2.Patients were performed the examinations of evaluation systems again 1 month after discharge,along with TDI score.According to the type of inhaled drugs,the patients were divided into three groups,which includes single therapy group(LAMA),dual therapy group(ICS/LABA,LABA/LAMA),and triple therapy group(ICS/LABA/LAMA).Changes in each evaluation system of the three groups were analyzed after maintenance treatment with different drugs.Results1.28 patients were enrolled.The analysis revealed that FEV1%pred was negatively correlated with mMRC,CCQ total score,SGRQ total score,SGRQ symptom score and activity score(r=-0.582,r=-0.593,r=-0.657,r=-0.572,r=-0.647,respectively,P<0.05),and positively correlated with BDI(r=0.607,P<0.05);CCQ functional state score was positively correlated with mMRC,SGRQ activity score(r=0.816,r=0.921,respectively,P<0.01),and negatively correlated with BDI(r=-0.891,P<0.01);CCQ symptom score was positively correlated with CAT and SGRQ symptom score(r=r=0.670,r=0.521,respectively,P<0.01);CCQ mental score was positively correlated with HDRS and SGRQ impact score(r=0.775,r=0.572,respectively,P<0.01);SGRQ activity score was positively correlated with mMRC(r=0.838,P<0.01),and negatively correlated with BDI(r=-0.912,P<0.01).SGRQ symptom score was positively correlated with BODE index and CAT(r=0.804,r=0.648,respectively,P<0.05);SGRQ impact score were positively correlated with mMRC and HDRS(r=0.637,r=0.574,respectively,P<0.01).2.Regarding the patients as a whole,the evaluation scores between pre-and posttreatment all improved except CCQ mental score and HDRS(P<0.05).3.The total score of CCQ in the single therapy group decreased,and the total score of CCQ and SGRQ in the triple therapy group improved(P<0.05).The total score of CAT,CCQ and SGRQ in the dual therapy group also decreased(P<0.05).Conclusion1.CAT,mMRC,BDI,CCQ,SGRQ,BODE effectively reflect varying aspects of patients’ symptoms,functional activities,quality of life and so on,to different extant.The evaluation system correlates with each other in different degrees,which makes it a supplementary choice of pulmonary function examination.In order to comprehensively evaluate the patient’s condition,clinicians are advised to apply one or more evaluation systems according to the actual situation;2.The evaluation system of HDRS should be used in the screening in patients with COPD for the diagnosis of depression;3.Each evaluation system of COPD changes in a varying degree when comes to different clinical therapies,so it can be routinely used to evaluate the efficacy of different clinical treatment.
