| Objective To observe the difference of short-term curative effect of cervical traction in different positions for cervical spondylotic radiculopathy with unstable tendency,and to provide individual traction reference for CSR with unstable tendency.Methods A total of 90 patients with cervical spondylotic radiculopathy who had been diagnosed as cervical spine instability in the outpatient department or in-patient department of the Spine Department of the First Affiliated Hospital of the Guangxi University of Chinese Medicine from January 2018 to June 2020 were selected for the study,the patients were divided into a group(45 cases in Group A)and a control group(45 cases in Group B).During the follow-up,45 cases in group A were treated by traction in flexion position or extension position,group A lost 2 cases,group B lost 4 cases,finally group A included 43 cases(17 male,26 female),Group B included 41cases(19 male,22 female),the overall age range of 20-55 years old,the average age of Group A was(38.84 ± 9.73)years old,the average duration of disease was(15.58 ± 4.41)months,the average BMI index was(23.49 ± 2.45)months,the average age of Group B was(37.68 ± 10.05)years,the average duration of disease was(14.61 ± 4.81)months,the average BMI index was(23.66 ± 2.35)months,and the number of unstable segments and cases were:Group A: C 3,10 cases,C 4,16 cases,C 5,15 cases,C 6,6 cases.Group B:C2,1,C3,9,c4,15,c5,14,C6,7.Results There was no significant difference in sex,course of disease,age and BMI between the two groups(P > 0.05).Before treatment,there was no significant difference between the two groups in Vas score,NDI score and symptoms scale of jingjiu cervical spondylosis in Tanaka(P > 0.05).Group comparison: after 3 days,7 days,14 days,1 month and 3 months of treatment,there were significant differences in Vas,NDI and symptoms scale of jingjiu cervical spondylosis between the two groups(p < 0.05),the scores of Vas and NDI at each time point after treatment were lower than those before treatment,and the scores of symptoms scale of jingjiu cervical spondylosis in Tanaka were higher than those before treatment There was significant difference between the two groups in the scores of Vas,NDI and Tanaka Jingjiu cervical spondylosis symptom scale at 3,7 days after treatment and 14 days,1 month and 3 months after treatment(p < 0.05)There was no significant difference(p > 0.05)between the two groups in the scores of Vas,NDI and Tanaka Jingjiu symptom scale at 14 days,1 month and 3 months after treatment.The scores of Vas and NDI were the lowest at 14 days after treatment,yasuhisa Tanaka had the highest score on the symptom scale of cervical spondylosis.There was no significant difference between Group A and group B(p = 0.193 >0.05)in the scores of Vas,NDI and Tanaka Jingjiu symptom scale before treatment.There was significant difference between Group A and group B(p< 0.05)in the scores of Vas,NDI and Tanaka Jingjiu symptom scale at 3,7 and14 days after treatment.The scores of Vas and NDI in group A were lower than those in Group B at 3,7 and 14 days after treatment,and the scores of symptoms scale of Jingjiu cervical spondylosis in Group A were higher than those in Group B.There was no significant difference between Group A and group B in the scores of Vas,NDI and Tanaka Jingjiu symptoms scale 1 month and 3 months after treatment(P > 0.05).Conclusions 1.Cervical traction in flexion,hyperextension and neutral position under the guidance of dynamic X-ray can effectively improve the clinical symptoms of cervical spondylotic radiculopathy.2.The short-term effect(3 days,7 days and 14 days)of flexion or hyperextension traction in the treatment of cervical spondylotic radiculopathy with a tendency to cervical instability is better than that of conventional neutral traction,and quickly into the work and daily life.3.After 14 days of treatment,the efficacy of both drugs decreased gradually over time. |
