The Clinical Curative Observation Of Yiqi Xuanfei Huayin Jiedu Decoction In Fei YU Shui Ting Syndrome Of Cute Respiratory Distress Syndrome

Objective:To observe the clinical effect of Yiqi Xuanfei Huayin Jiedu decoction on Fei YU Shui Ting syndrome of Cute Respiratory Distress Syndrome,to explore the pathogenesis of Yiqi Xuanfei Huayin Jiedu Decoction in improving the clinical symptoms of ARDS patients,and to provide a new idea for the diagnosis and treatment of ARDS in traditional Chinese and western medicine.Methods:A total of 70 patients with mild to moderate ARDS who were hospitalized in the Department of Critical Care,the Second Department of ICU and the Emergency Department of the East Affiliated Hospital of Shandong University of Traditional Chinese Medicine from December 2018 to December 2020 were included in this study.The patients were randomly divided into two groups,control group and treatment group by random number table method.There were 36 cases in the treatment group and 34 cases in the control group.Cases that met the exclusion criteria during the study were excluded.A total of 60 cases were finally included in the data analysis and discussion,which were divided into the treatment group and the control group,with 30 cases in each group.According to the clinical guideline of mechanical venti’lation for adult ARDS issued by American Thoracic Association European Society of Critical Care Medicine and American Society of Critical Care Medicine in 2017,both groups were given routine Western Medicine Diagnosis and Treatment.The treatment group was given nasal feeding of Yiqi Xuanfei Huayin Jiedu decoction for 7 days,and the control group was given nasal feeding of the same amount of warm boiled water for 7 days.By recording the TCM symptom scores before and after treatment in patients with 7 days,SOFA score,Soluble thrombomodulin(sTM)and daily arterial-alveolar oxygenpartialpressureratio(a/APO2),Extravascular lung water content index(EVLWI),compared to patients with 7 days after SOFA score and total mechanical ventilation time,to observe the clinical effect and significance of Yiqi Xuanfei Huayin Jiedu decoction in treating ARDS syndrome of Fei Yu Shui Ting.Results:The TCM symptom score,SOFA score and Soluble thrombomodulin(sTM)in the treatment group and the control group were improved after treatment.After 4 days of treatment,there were,statistically significant differences between the control group and the treatment group in Extravascular lung water content index(EVLWI)and arterial-alveolar oxygenpartialpressureratio(a/APO2),P<0.01;After 7 days of treatment,TCM symptom score,SOFA score,Soluble thromboregulatory protein(STM)in the treatment group were better than those in the control group,and P<0.001,showing statistically significant differences.The final comparison of total mechanical ventilation time between the two groups was P<0.001,and there was a statistically significant difference.Conclusion:On the basis of conventional Western medicine diagnosis and treatment,the addition of Yiqi Xuanfei Huayin Jiedu decoction can improve the clinical symptoms and prognosis of ARDS patients with mild-to-moderate of Fei Yu Shui Ting syndrome,reduce the pulmonary edema,reduce the patients with pulmonary vascular endothelial damage degree,improve the level of oxygen and oxygen in patients,reduce eventually to systemic organ dysfunction in patients with the chance of development.
