Eearly Warning Of Vascular Dementia Based On Gross Body Movements During Sleep

Objective:The patients with vascular cognitive impairment(Vascular Cognitive Impairment,VCI)were monitored continuously by sleep pressure sensor and the quantitative data of gross body movement during sleep were collected to explore the relationship between gross body movement and disease severity and sleep quality in patients with VCI,so as to provide a new reference for the early warning and evaluation of vascular dementia(Vascular dementia,Va D).Methods:Fifty-five VCI patients who were hospitalized in the Department of Neurology in the affiliated Hospital of Shandong University of traditional Chinese Medicine and the second affiliated Hospital of Shandong University of traditional Chinese Medicine from January 2020 to December 2020 were selected as subjects,and 30 healthy volunteers without sleep disorders were recruited in the same period.According to the severity of vascular cognitive impairment,55 patients with VCI were divided into mild VCI group(n = 30)and Va D group(n = 25).The sleep pressure sensor was used to collect the data of large body movement during sleep,and the Pittsburgh Sleep quality scale(Pittsburgh Sleep Quality Index,PSQI)and Epworth sleepiness scale(Epworth Sleepiness Scal,ESS)were scored at the same time.The clinical data,general movement and sleep quality score data of each group were compared,and the correlation between gross movement and the severity of cognitive dysfunction and sleep quality in patients with VCI was analyzed.Results:(1)There was no significant difference in gender composition,age,BMI index and years of education between mild VCI group,Va D group and healthy control group(P>0.05).(2)With the increase of age,the total body movement decreased during sleep in both VCI group and healthy control group.(3)The scores of mini mental state scale(Mini-Mental State Examination,MMSE)and Montreal cognitive assessment scale(Montreal Cognitive Assessment,Mo CA)in VCI group were significantly lower than those in healthy control group,and the scores of MMSE and Mo CA in Va D group were significantly lower than those in mild VCI group.The course of disease in Va D group was significantly longer than that in mild VCI group.There was no significant difference in ADL score between mild VCI group and healthy control group(P > 0.05).The ADL score of mild),Va D group was significantly higher than that of mild VCI group(P<0.05).(4)The gross movement of VCI patients was significantly lower than that of the control group,and there was significant difference between the two groups(P<0.05).The gross exercise times of Va D group < mild VCI group<healthy control group.(5)The PSQI score of patients with VCI was significantly higher than that of the control group,and there was significant difference between the two groups(P<0.05).The PSQI score of Va D group > mild VCI group > healthy control group.(6)The ESS score of patients with VCI was significantly higher than that of the control group,and there was significant difference between the two groups(P<0.05).ESS score of Va D group> mild VCI group > healthy control group.(7)The severity of cognitive impairment in patients with VCI was negatively correlated with general movement(r=0.702,P<0.05),and positively correlated with PSQI score(r=0.719,P<0.05)and ESS score(r=0.682,P<0.05).(8)The gross movement of VCI patients was negatively correlated with the course of disease(r=0.843,P<0.05),and positively correlated with MMSE score(r=0.898,P<0.05)and Mo CA score(r=0.898,P<0.05).(9)The PSQI score of VCI patients was positively correlated with the course of disease(r=0.735,P<0.05),and negatively correlated with MMSE score and Mo CA score(r=0.763,P<0.05).(10)The ESS score of VCI patients was positively correlated with the course of disease(r=0.454,P<0.05),and negatively correlated with MMSE score(r=0.453,P<0.05;r =0.494,P<0.05).(11)There was a negative correlation between gross movement and PSQI score in patients with VCI(r=0.704,P<0.05).Conclusions:(1)The longer the course of VCI is,the more serious the condition is,the sleep quality at night is significantly decreased,and the daytime sleepiness is significantly increased,and there is a significant correlation between sleep quality and the severity of cognitive impairment.(2)The longer the course of VCI is,the more serious the disease is and the less general movement is,and there is a significant correlation between gross movement and the severity of cognitive impairment during sleep.(3)There is a negative correlation between total body movement and nocturnal sleep quality in patients with VCI.
