| Objectives:1.The positive rate of Malassezia in acne patients and acne patients of different degrees and genders were counted,and the relationship between Malassezia and acne was analyzed.2.To seek the best treatment for Malassezia positive acne patients by comparing the therapeutic efficacy of positive and negative patients,as well as the therapeutic efficacy of plus or without antifungal therapy in positive patients.Methods:1.Retrospective analysis was performed on 285 cases of acne patients who received fungal microscopic examination in our department from December 2019 to October 2020.Gender,age,clinical photos and microscopic examination results were collected,and Excel 2013 was used to organize the above data,and SPSS22.0 was used for statistical analysis.2.Clinical pictures of 118 acne patients before and after treatment were collected and divided into three groups:group A(51 cases)(Malassezia negative,treated with isotretinoin),group B(26 cases)(Malassezia positive,treated with isotretinoin),group C(41 cases)(Malassezia positive,treated with isotretinoin+itraconazole).The curative effect before and after treatment of group A and B,B and C was compared to analyze whether there was correlation between Malassezia infection and acne treatment efficacy,and whether systemic antifungal therapy was necessary on the basis of systemic administration of isotretinoin in acne patients infected with Malassezia.Results:1.Among 285 acne patients,the total positive rate of Malassezia was 47.02%,40.33%in mild patients,58.33%in moderate patients,and 90%in severe and extremely severe patients.The positive rate was 64.46%in male patients and 34.15%in female patients.Chi-square test showed that the positive rate of male and female was significantly different(P<0.05).There were significant differences in the positive rate between mild and moderate(P<0.05),between moderate and severe and extremely severe(P<0.05),and between mild and severe and extremely severe(P<0.05).2.After 1 month,2 months and 3 months of treatment,the difference between the GAGS score of group A and group B and the initial GAGS score was respectively compared.Non-parametric test showed that there was no statistical significance in the difference between the two groups after 1 month,2 months and 3 months of treatment.After 1 month,2 months and 3 months of treatment,the differences between the GAGS scores of group B and group C and the initial GAGS scores were respectively compared.Non-parametric test showed that there was no statistical significance in the differences between the two groups after 1 month,2 months and 3 months of treatment.The effective rates of group A after 1 month,2 months and 3 months were 33.33%,70.59%and 92.16%,respectively;the effective rates of group B after 1 month,2 months and 3 months were 30.77%,76.92%and 96.15%,respectively;the effective rates of group C after 1 month,2 months and 3 months were 29.26%,53.66%and 95.12%.The effective rates of group A and group B after 1 month,2 months and 3 months were respectively compared.The chi-square test showed no statistically significant difference in the effective rates between the two groups after 1 month,2 months and 3 months.The effective rates of group B and group C after 1 month,2 months and 3 months were respectively compared.The chi-square test showed no statistically significant difference in the effective rates between the two groups after 1 month,2 months and 3 months of treatment.Conclusions:1.The total detection rate of Malassezia was 47.02%,64.46%in males and 34.15%in females.The detection rate of male patients was higher than that of female patients,and the more severe the disease,the higher the detection rate.2.In acne patients complicated with Malassezia infection,it has a good curative effect to take isotretinoin for treatment,and on the basis of systemic administration of isotretinoin,there is no need for systematic antifungal therapy,and the curative effect obtained is not different from that of addition of antifungal therapy,nor is the curative effect of taking isotretinoin in patients with negative malassezia.3.No matter whether patients with acne are complicated with Malassezia infection or not,isotretinoin treatment can achieve a relatively satisfactory effect,but isotretinoin treatment should not be less than 3 months. |
