Professor Lin Dingkun’s Experience In Treating Nerve Root Cervical Spondylopathy With Decoction

ObjectiveThrough data mining,the use of decoctions in the past literature and the use of decoctions by Professor Lin Dingkun to treat radiculopathy of cervical spondylosis,the distribution of syndromes,medication and prescription rules,in order to provide references for clinical prescriptions and medications,and better develop the advantages and characteristics of traditional Chinese medicineMethods1.Apply data mining technology,use the full-text database of Chinese academic journals(CNKI,Wanfang,and Weipu)as the source of data and information,and use"nerve radiculopathy" combined with "Chinese medicine" or "Chinese medicine" or"integrated Chinese and Western medicine" or "syndrome differentiation" or "syndrome type" as the subject of the search.The search time is set from January 1,2000 to January 1,2020.The relevant literature on the treatment of cervical spondylotic radiculopathy with oral Chinese medicine is obtained,and the database is established and carried out Statistical Analysis.Use ancient and modern medical records cloud platform,Excel,IBM SPSS 26.0 statistical software,and Clementine 12.0 to conduct frequency statistics of syndrome types,frequency statistics of traditional Chinese medicines,analysis of characteristics and flavors,analysis of association rules,and cluster analysis.2.By selecting 103 cases of cervical spondylotic radiculopathy who were diagnosed and treated by Professor Lin Dingkun in the outpatient clinic of Guangdong Provincial Hospital of Traditional Chinese Medicine from January 1,2017 to January 1,2020,and met the screening criteria.Use ancient and modern medical records cloud platform,Excel,IBM SPSS 26.0 statistical software,and Clementine 12.0 to perform frequency statistics on syndrome types,frequency statistics on traditional Chinese medicines,analysis of characteristics and flavors,analysis of association rules,and cluster analysis.Results1.Include 51 documents that meet the criteria,and the distribution of syndrome types is from more to less:Qi stagnation and blood stasis type,wind-cold dampness type,wind-cold blockage type,qi deficiency and blood stasis type,insufficient qi and blood,liver and kidney deficiency type,Liver and kidney deficiency and qi stagnation type,liver and kidney deficiency,wind,cold and dampness type,are mainly based on evidence.A total of 42 prescriptions are included,including 31 self-made prescriptions,11 classic prescriptions,and 103 traditional Chinese medicines.The most frequently used clinical Chinese medicines are Rhizoma Ligustici Chuanxiong,cassia twig,licorice,Angelica sinensis and other 13 flavors.The meridians of the medicines are mainly liver,spleen,heart,and lungs;the medicine tastes mainly pungent,sweet and bitter,and the medicinal properties are warm,Mainly flat and lukewarm;the TCM combinations with the highest drug pair correlation are Rhizoma Ligustici Chuanxiong and Angelica sinensis,Guizhi and Kudzu root,Rhizoma Ligustici Chuanxiong and Qianghuo.The cluster analysis divides the Chinese herbs into 4 effective cluster groups,namely Angelica sinensis,Dilong,Safflower;Kudzu root,Radix Paeoniae Alba,Caulis spatholobi,Cassia twig;Gentiana,Weilingxian,Licorice;Qianghuo,Alpinia nigra,Rhizoma Ligustici Chuanxiong.2.Include 103 medical records that meet the criteria,with Qi-blood differentiation from more to less:Qi deficiency and blood stasis type,Qi deficiency type,blood stasis type,Qi stagnation and blood stasis type,blood deficiency type,Qi and blood deficiency type;The frequency of occurrence in the syndrome differentiation is ranked as follows:wind,wind,cold and dampness,dampness,fire,phlegm,dampness,rheumatism and heat,and the frequency of occurrence in the syndrome differentiation of qi and blood is ranked from most to least to deficiency of qi A total of 27 prescriptions for blood stasis combined with dampness,qi deficiency combined with wind-cold dampness,etc.involved a total of 13 self-made prescriptions,and 14 classic prescriptions.A total of 133 Chinese medicines have been used.The commonly used Chinese medicines used in the clinic with frequency of ≥ 25 times are Angelica sinensis sinensis,Astragalus,Licorice,Kudzu root,Rhizoma Ligustici Chuanxiong,etc.14 flavors.The meridians of the medicines used are mainly the 3 meridians of spleen,liver and lung;Mainly pungent and bitter,and the medicinal properties are mainly warm,flat slightly cold,and lukewarm;the combination of Chinese medicines with the highest drug pair correlation is Astragalus and Angelica sinensis,Angelica sinensis and Astragalus,Kudzu root and Astragalus,cluster analysis divides the Chinese medicine into 4 Effective clustering group,Rhizoma Ligustici Chuanxiong,red peony,earthworm,achyranthes,safflower,astragalus,Angelica sinensis;Angelica sinensis dahurica;Kudzu root Atractylodes,Bupleurum;licorice,tangerine peel,white peony.ConclusionThis study found that the pathogenesis of radiculopathy of cervical spondylosis is mainly based on deficiency and deficiency of excess,the deficiency of deficiency is mainly due to deficiency of lung and spleen Qi and insufficient liver blood.The symptoms are mostly wind-cold dampness evil and blood stasis,and the disease is in the liver and spleen.,It is especially important to protect the spleen and stomach.In terms of medication,according to internal factors,Astragalus.Atractylodes macrocephala,etc.are used to replenish qi and solidify the surface,invigorate the spleen and dispel dampness,and use Chinese Angelica sinensis and white peony root to nourish the blood and strengthen the muscles,so that the righteous qi is sufficient.For exogenous pathogens,use Kudzu root root,Guizhi Zhiji release,Qianghuo,Angelica sinensis dahurica to dispel cold and dehumidification;for qi and blood loss factors,add qi and blood activating drugs,such as Bupleurum,red peony,Rhizoma Ligustici Chuanxiong,and Di Dragon,Caulis spatholobi,etc.,the method of reconciling qi and blood runs through the disease throughout the treatment process.
