| Objective:To investigate diagnostic and prognostic value of s PD-1 on patients who have sepsis in pediatric intensive care unit.Method: Subjects were children with sepsis admitted to our PICU from January 2018 to July 2019.All the children were divided into the sepsis group and the severe sepsis group according to the conditions admitted to the PICU.Blood samples of all children were collected at the time of admission.Basic information of the children,such as age and gender,as well as clinical indicators such as blood routine within 24 hours of admission to the PICU,were collected.PSOFA score was performed for all the children.All discharged patients were followed up by telephone for 28 days.According to the follow-up survival status,the children were divided into survival group and death group.Serum s PD-1 was detected by enzyme-linked immunosorbent assay.Finally,the correlation statistical analysis was conducted.Results :A total of 64 children were enrolled.There were 31 patients in the sepsis group(48.4%)and 33 patients in the severe sepsis group(58.6%).Serum between the two groups the s PD-1 [severe sepsis group:440.57(269.43,924.14)pg/ml,sepsis groups: 290.63(137.18,465.82)pg/ml,P < 0.05)],PCT [severe sepsis group: 2.00(0.44,45.84)ng/ml,sepsis groups:0.65(0.22,2.00)ng/ml,P < 0.05)],p SOFA [severe sepsis groups: 3.0(1.5,5.0),sepsis groups: 1.0(0,1.0),P < 0.01)] level are different.There were 55 patients(85.9%)in survival group and 9 patients(14.1%)in death group.The following indexes in the death group were higher than those in the survival group: serum s PD-1[death group:742.5(571.26,1098.70)pg/ml,survival group: 300.16(166.51,482.25)pg/ml,P < 0.01],PSOFA [death group: 7.0(2.5,11.5),survival group: 1.0(0,2.0),P < 0.05],procalcitonin [death group:19.84(1.89,100.00)ng/ml,survival group:0.79(0.23,2.67)ng/ml,P < 0.01)].ROC curve analysis assessed the area under the curve for28-day mortality judgment of s PD-1,PSOFA score,PCT were 0.743,0.879,0.790,respectively.The cut-off point value of s PD-1 was 489.59pg/ml.Logistic regression analysis showed that s PD-1 was an independent predictor of child mortality.Conclusion:1.Serum s PD-1 level is correlated with the condition and prognosis of children who have sepsis in pediatric intensive care unit.The more severe the disease,the worse the prognosis,and the higher the level of s PD-1.2.Serum s PD-1level was an predictor of 28-day mortality in children with who have sepsis in pediatric intensive care unit. |
