| Objective:The shunting of high-risk HPV E6/E7 mRNA and HPV DNA to ASCUS patients is currently widely used in clinical practice.The article uses meta-analysis to systematically evaluate the diagnostic efficacy of HPV E6/E7 mRNA and HPV DNA shunting ASCUS patients,which is a clinical shunt ASCUS patients provide more accurate evidence-based medicine.Methods:By searching 8 databases including China Knowledge Network(CNKI),China Biomedical Literature Database(CBM),Wanfang,Weipu,Pubmed,Embase,Web of Science,and Cochrane,the search time is self-built until November 2020.Cervical biopsy was used as the gold standard,cinii+or HSIL+as the critical value.According to the inclusion and exclusion criteria,the literatures were screened by Noteexpress software,and the QUAD AS in Review Manager 5.3 was used The quality of literature was evaluated by using stata 15.1 software.Results:HPV E6/E7mRNA diagnosis of CINII+in ASCUS patients:combined sensitivity was 0.858(95%CI:0.836-0.879),combined specificity was 0.597(95%CI:0.582-0.611),combined positive likelihood ratio was 2.375(95%CI:2.036-2.770),the combined negative likelihood ratio is 0.258(95%CI:0.216-0.309),the combined AUC is 0.8535,and the Q index is 0.7845.The combined diagnostic odds ratio DOR was 10.235(95%CI:7.507-13.955).HPV DNA diagnosis of CINII+in ASCUS patients:combined sensitivity was 0.912(95%CI:0.893-0.929),combined specificity was 0.401(95%CI:0.387-0.416),combined positive likelihood ratio was 1.551(95%CI:1.381-1.742),the combined negative likelihood ratio was 0.269(95%CI:0.217-0.334),the combined AUC was 0.8204,and the Q index was 0.7537.The combined diagnostic odds ratio DOR was 6.594(95%CI:4.797-9.062).Conclusions:The sensitivity,positive likelihood ratio and negative likelihood ratio of HPV E6/E7 mRNA and HPV DNA shunt ascus patients were not significantly different,but the specificity of HPV E6/E7 mRNA in detecting cinII+ in ascus patients was significantly higher than that of HPV DNA,suggesting that HPV E6/E7 mRNA shunt ascus patients has better diagnostic value than HPV DNA. |
