Objective : To investigate the relationship between vertebral-basilar artery blood flow and posterior circulation infarction. Methods : A total of 98 patients admitted to the First Affiliated Hospital of Xinjiang Medical University from December 2019 to August2020 with definite diagnosis of posterior circulatory infarction were collected as the case group, and 110 patients with non-posterior circulatory infarction hospitalized during the same period were selected as the control group. All enrolled patients underwent head diffusion weighted imaging(DWI),cervical vascular ultrasound, transcranial Doppler imaging(TCD)and head and neck CT angiography(CTA).The diameter of the extracranial vertebral artery, internal carotid artery and common carotid artery, systolic blood flow velocity, diastolic blood flow velocity and mean blood flow velocity measured by cervical vascular ultrasound were collected, collect the TCD of bilateral vertebral artery intracranial segment, basal artery, posterior cerebral artery systolic blood flow velocity and diastolic blood flow velocity and mean blood flow velocity,According to the three-dimensional reconstruction of head and neck CTA,the mean diameters of V5 segment of vertebral artery and basilar artery were measured, and the blood flow per minute of corresponding vessels was calculated by the calculation formula. The above examination results of the two groups of patients were compared and analyzed. Results: 1.The diameter of the intracranial segment of the vertebral artery in the case group was smaller, the blood flow velocity was slower and the blood flow was lower than that in the control group.2.Compared with the control group, the blood flow velocity of basilar artery in case group was slower and the blood flow was lower.3.The diastolic and mean velocity of the right common carotid artery in the case group was slower than that in the control group.4.The ROC curve suggested that the decrease of intracranial blood flow in the vertebrobasilar artery system had a certain predictive effect on the occurrence of posterior circulation infarction. Conclusion : There is a close relationship between decreased vertebral-basilar blood flow and posterior circulation infarction.TCD and head and neck CTA can be used to measure the blood flow of vertebro-basilar artery to predict the occurrence of posterior circulation infarction to a certain extent.
