| Objective:To observe the efficacy of needle knife therapy combined with exercise in the treatment of knee osteoarthritis,and to evaluate the muscle performance of quadriceps femoris by muscle performance tester,so as to analyze the objective indexes that can evaluate the curative effect of knee osteoarthritis.and to provide new ideas for the treatment of knee osteoarthritis.Methods:This study included 120 patients with knee osteoarthritis who came to the orthopaedic outpatient department of Guangdong Second Hospital of traditional Chinese Medicine(2019.07-2020.12).They were randomly divided into experimental group(n=40),control group A(n=40)and control group B(n=40).Among them,the scheme of the experimental group was needle knife therapy combined with exercise,the control group A was treated with a needle knife therapy,and the control group B was treated with exercise.Pain evaluation,joint function evaluation and muscle performance evaluation were performed in all patients before treatment.The observation indexes included VAS score,WOMAC score,muscle tone and muscle stiffness.The observation indexes of the patients were evaluated again after one course of treatment(6 weeks).Results:In the end,there were 5 cases drop-out in the experimental group,6 cases drop-out in the control group A and 7 cases drop-out in the control group B.There was no significant difference in age,sex ratio,course of disease,BMI,VAS,WOMAC,muscle tone and muscle stiffness among the three groups.VAS score:the VAS score of the three groups after treatment was significantly lower than that before treatment(P<0.01).After treatment,the),VAS score of the experimental group was significantly lower than that of the control group A and the control group B(P<0.05),and the difference was statistically significant.There was no significant difference in VAS between control group An and control group B after treatment(P>0.05).WOMAC:the WOMAC score of the three groups after treatment was significantly lower than that before treatment(P<0.01).Before and after treatment,the difference of WOMAC score in experimental group was significantly lower than that in control group A and control group B(P<0.05),and the difference was statistically significant.After treatment,there was no significant difference in WOMAC score between control group An and control group B before and after treatment.(P>0.05)Muscle tone:the medial femoral muscle tone of the three groups after treatment was significantly lower than that before treatment(P<0.01).There was no significant difference in muscle tone among the three groups after treatment or before and after treatment(P>0.05).Muscle stiffness:there was no significant change in the muscle stiffness of medial vastus muscle in the three groups after treatment(P>0.05).Adverse reactions:there were no serious adverse reactions in the three groups of patients in the experimental stage.There were 2 cases of mild subcutaneous hematoma in the experimental group and 3 cases in the control group A.Conclusion:1.Needle knife therapy combined with exercise can effectively improve the pain and joint movement function of KOA patients,and it is more effective than needle knife treatment group or exercise group alone.2.In patients with unilateral knee osteoarthritis,the muscle tension of the medial vastus muscle on the affected side was significantly higher than that of the medial vastus muscle on the healthy side.3.While the clinical symptoms were improved,the muscle tone also decreased to a certain extent.4.It is feasible to evaluate the clinical efficacy of knee osteoarthritis by measuring muscle performance by MyotonPRO. |
