Objective:Objective:To study the biomechanical properties of the cornea in patients with congenital blepharoptosis,and to explore the changes in the biomechanical properties of the cornea after correction of ptosis.Methods:A retrospective study collected 72 cases of patients aged 5-18 years who were diagnosed with congenital ptosis in the ophthalmology department of the Second People’s Hospital of Yunnan Province from June 2019 to January 2021(60 cases in one eye and 12 cases in both eyes)),and at the same time collect 60 cases of 5-18 years old healthy control group.Corvis ST corneal biomechanical analyzer was used to record the A-timel,A-length1,V-in,A-time2,A-length2,V-out,HC-time,PD,HC-radius,DA,IOP,bIOP,CCT and other corneal biomechanics of patients with unilateral congenital ptosis of the ptotic eyes and the Fellow eyes,the healthy control group,and the postoperative ptosis correction,for comparative analysis.Results:1.In patients with unilateral congenital blepharoptosis,the ptotic eye,the Fellow eye,and the healthy control group have statistical differences in the second applanation time,maximum indentation time,and central corneal thickness:(1)A-time2:The difference in A-time2 between the ptotic eyes and the Fellow eyes is not significant(p=0.991>0.05),the difference in A-time2 between the ptotic eyes and he healthy control group is significant(p=0.001<0.05),the A-time2 difference between the Fellow eyes and the healthy control group was significant(p=0.001<0.05).healthy control group(21.06±0.32ms)>Fellow eyes(20.77±0.57ms)>ptotic eyes(20.76±0.52ms);(2)HC-time:The difference in HC-time between the ptotic eyes and the Fellow eyes is not significant(p=0.505>0.05),and the difference in HC-time between the ptotic eyes and the healthy control group is Significant(p=0.001<0.05),the HC-time value of difference between the Fellow eyes and the healthy control group was significant(p=0.010<0.05).healthy control group(16.46±0.44ms)>Fellow eyes(16.25±0.48ms)>ptotic eyes(16.20±0.39ms);(3)CCT:There is a significant difference in CCT between the ptotic eyes and the Fellow eyes(p=0.008<0.05),the CCT difference between the ptotic eyes and the healthy eye was significant(p=0.005<0.05),and the CCT difference between the Fellow eyes and healthy control group was not significant(p=0.861>0.05).the ptotic eyes(559.47±34.43um)>the Fellow eyes(543.25±31.53um)>healthy control group(542.18±33.83um);2.V-in,A-length2,A-time2,PD between the different severity of unilateral congenital ptosis patients,is statistically significant(p<0.05),and the difference is compared by means:V-in(0.12±0.02 m/s)in the mild to moderate group>V-in(0.11±0.01 m/s)in the severe group,p=0.048<0.05;A-length2(2.03±0.34mm)in the severe group>light-moderate group(1.85±0.29mm),p=0.049<0.05;A-time2(20.96±0.45ms)in the mild-moderate group>Severe group(20.67±0.54ms),p=0.046<0.05.PD in the mild to moderate group(4.62±0.33mm)>severe group(4.39±0.33mm),p=0.013<0.05;3.There is no significant correlation between gender and the biomechanical parameters of the ptotic eyes in patients with unilateral congenital ptosis(p>0.05);4.There was A significant positive correlation between age and V-in,PD,DA in the ptotic eyes of patients with unilateral congenital ptosis(r>0,P<0.05),and A significant negative correlation between age and A-length2,V-out(r<0,P<0.05);5.There is a significant positive correlation between age and the overall PD and DA of patients with congenital ptosis of the ptotic eyes,the Fellow eyes,and the healthy control group(r>0,p<0.05);6.After correction of congenital blepharoptosis,A-timel(7.96±0.49ms)>preoperative(7.70±0.51ms),p=0.019<0.05,the difference was statistically significant;Postoperative IOP(19.19±3.17)mmHg)>preoperative(17.77±3.15mmHg),p=0.039<0.05,the difference is statistically significant.Conclusions:1.The cornea of patients with unilateral congenital ptosis is more easily deformed than healthy eyes under the same external force;2.After the correction of ptosis,the degree of deformation of the cornea after the force is improved compared with that before the operation;3.There is a significant correlation between age and the corneal biomechanics properties of the ptotic eyes in patients with congenital ptosis;4.In patients with unilateral congenital ptosis,the CCT of the ptotic eyes and the Fellow eyes is thicker than that of the healthy control group.
