Objective:To explore the correlation between single nucleotide polymorphism of carcinoembryonic antigen-associated adhesion molecule 1(CEACAM1)gene(rs555643516,rs927180583,rs971040608)and the incidence of dilated cardiomyopathy(DCM)in Han population in Yunnan.Methods:A total of 59 patients diagnosed with dilated cardiomyopathy in the Department of Cardiology,the First Affiliated Hospital of Kunming Medical University from 2018 to 2020 were selected as the experimental group and 14 healthy subjects as the control group.The general medical history,age,gender,weight,height,blood pressure and other clinical data of the enrolled subjects and NYHA classification were recorded.The Body Mass Index(BMI)was calculated,Fasting venous blood was extracted from the subjects in the laboratory of our hospital to measure the patients’fasting blood glucose,Low density lipoprotein(LDL),triglyceride,total cholesterol,High density lipoprotein(HDL),blood coagulation function.In addition,in our department,we completed the five components of heart and lung(including troponin I,myoglobin,CK-MB,BNP,and D-dimer),and completed the heart ultrasound,and recorded the Inter ventricular septum thickness of the subject(IVSD),Left ventricular end diastolic dimension(LVEDD),Left ventricular posterior wall end diastolic diameter(LVPWD),Right ventricular diameter(RVD),Left atrium diameter(LAD)and left ventricular ejection fraction(LVEF).Venous blood was collected to extract DNA from patients,and CEACAM1 gene was sequenced for second-generation analysis to analyze Single nucleotide polymorphism(SNP)of patients.SPSS26.0 was used to analyze the differences in the general clinical data,genotype frequency and allele frequency of CEACAM1(rs555643516,rs927180583,rs971040608)between patients with dilated cardiomyopathy and healthy controls.Binary Logistic analysis was used to analyze the differences in gender,age,BMI,smoking history,drinking history and other clinical data of the subjects.Result:1.The average age,BMI,smoking history,CK-MB,ATNI,BNP,D-dimer,pulmonary arterial pressure,IVSD,LVPSD,LVEWD,LA and LVEF of the DCM group were significantly different from those of the healthy control group.Blood pressure,drinking history,fasting blood glucose,heart rate,triglyceride,low density lipoprotein,total cholesterol,high density lipoprotein and other differences were not statistically significant.2.The distribution of different genotypes of CEACAM1(rs555643516,rs927180583,rs971040608)in DCM group and healthy control group all complied with Hardy-Weinberg equilibrium law test(P>0.05),indicating that the sample population selected in this experiment was relatively representative.3.Compared with the healthy control group,the distribution of CEACAM1 genotype(rs555643516,rs927180583,rs971040608)in the DCM group was statistically significant,and the χ2 value of rs555643516 group was 6.095,P=0.047;χ2 value of rs927180583 was 6.635,P=0.036.The χ2 value of rs971040608 was 6.715,P=0.035,and there was no significant difference in allele distribution among different CEACAM1 genotypes(rs555643516,rs927180583,rs971040608).4.There was no significant difference between CEACAM1(rs555643516,rs927180583,rs971040608)gene polymorphism and BNP,NYHA cardiac function grade,LVEF and heart size in patients with dilated cardiomyopathy.5.The comparison of rs927180583 gene normal group(GG)and mutation group(GA+AA)in patients with dilated cardiomyopathy showed statistically significant differences in pulmonary arterial pressure.6.In this study,multivariate binary logistic regression analysis showed that gender,BMI,smoking history and drinking history were not independent influencing factors for DCM,while age might be an independent influencing factor for DCM,which was positively correlated with the incidence of DCM(P=0.008,OR value 1.311,95%CL 1.073-1.602).Conclusion:1.Polymorphism of CEACAM1 gene(rs555643516,rs927180583,rs971040608)may be associated with the incidence of DCM.2.Mutation of rs927180583 gene in patients with dilated cardiomyopathy may be related to elevated pulmonary artery pressure.
